Hi there,I would suggest a smallish implant 200-240 ccin a Bi Planar Partial submuscular pocket.This would hold the implant in the correct postion and then I would perform the mastopexy to lift the Nipple about 2 cm.The advantage of placing the implant partial submuscular is to avoid bottoming out or drooping of the breast after the procedure.

You have a moderate degree of drooping within the Breast.I would suggest a 300cc implant in partial submuscular plane (BiPlanar). This will allow the implant to be supported by the muscle and reduce the risks of developing rapid drooping of the breast.If you develop drooping then a more limited mastopexyor lift could be done secondarily.
