Breast augmentation after breast feeding
You should wait at least 3 months before a breast augmentation. But I like to allow up to 6 months of breast settling after breast feeding is completed.
This allows better evaluation of the breasts because they have stabilized.
In addition, weight loss after will also effect your breasts.
How long breast implants last
I tell my patients that once you have a breast implant you will need another surgery in the future, somewhere between 8-10 years.
Implants can encapsulate, shift, fail and the breast will age and sag and you will need another surgery for the above.
Mastopexy, no implant
To address your concerns of droopiness and make your breasts perkier will require a full mastopexy/small reduction. Yes, they will look smaller, but usually they are not tiny and are more proportionate. Implants have no role here as you have plenty of breast tissue.
You should wait until you are done having kids as a mastopexy is likely to affect your ability to breast-feed. Satisfaction rates for this procedure are very high and I encourage you to take you time and look into it.
To lift or not lift that is the question! Breast lift and augmentation.
To lift or not lift — that is the question! It is such a common issue for women who have been pregnant, lost breast volume and the skin has stretched. We all want a great result and I believe you can have an amazing result, but you must have a frank discussion with your plastic surgeon about the what would serve you best.
It is a big mistake to place an enormous implant in a misquided attempt to “lift” the breast.
Breast Implants without Lift
You will benefit from both a lift and implants. Implants alone will not be enough to achieve a lift. Be sure you consult with a board certified plastic surgeon for best results.
Only if you develop contracture
Silicone implants, especially low-bleed types used now, do not necessarily harden with time. We are uncertain what exactly causes hardening, or contracture, in some patients. This is essentially when a thick scar forms around the implant and causes the breast to get hard.
It’s up to you and your plastic surgeon.
There are several advantages to both subglandular and submuscular breast implant placement, and what’s right for one woman may not be right for another. Given that you want a more natural look, placing breast implants under the muscle may be the option that works best for you.
Subglandular placement may work better with your weightlifting routine though. The best answer will come from an experienced plastic surgeon who specializes in breast augmentation and can evaluate you in person and have a detailed discussion with you about your goals and the pros and cons of both placements.
Afraid breast implant will be too big
Many women with a thin build, such as you, are afraid that the implant will look too large. Afterward, most of those same women say they wish they had gone larger. There is not much difference in the appearance of a 300 cc and a 325 cc implant, especially for a patient who is 5′ 6″.
Predicting post operative results after a breast augmentation
I believe that you would get a nice result from a breast augmentation but you will not have the same result as the model’s photograph that you have included. A breast augmentation will give you a bigger version of yourself.
In our office we utilize a 3D camera system that will give you a very accurate image if what you would look like after surgery.
Breast lift needed
Placing implants over the muscle to achieve a breast lift is usually a futile maneuver. As long as you understand that scars are necessary for a breast lift, and without a lift you will have larger droopy breasts, you may proceed with breast augmentation alone.
All gel implants are cohesive not just “gummy bear”
There has been a lot of confusing terminology and a lot of debate about the various types of implants. For starters, all silicone gel implants sold in the U. S. are cohesive, which means that the gel holds together as a semi-solid.
So you could slice them open and the gel doesn’t flow out. The “gummy bear” implants, correctly called “form-stable” implants (Mentor CPG and Allergan Style 410) have a firmer gel gel so they can be molded into various shapes other than round.
These are the ones that are not yet available in the U. S but experts seem to agree that they are an advantage over the round gel implants only for a small percentage of patients.
They do have a long experience with them in Brazil but it is a long way to go for implants that may or may not be better for your specific needs.
Generally, Breast Implant Replacement is Through the Original Incision
In general, breast implant revision or replacement is done through the previous incision. In your case, this would be an areolar/ nipple incision. There is a slightly increased risk of capsular contracture through an areolar incision, due to a slightly higher bacterial contamination count through the areola versus the breast fold or underarm incision.

Your old incision should be big enough for pretty much any silicone breast implant. If your plastic surgeon is balking at the incision size, he/ she can check out the Keller funnel, a device that is similar to a pastry icing applicator made just to put breast implants into a smaller incision with less surgeon effort.
Implant profile
Choosing the correct profile for an implant depends on many factors. Perhaps the most important is the final appearance of the breast you are looking for. Rather than trying to pick an implant before surgery, consider taking a photograph of a similar sized and shaped woman after surgery to help guide your surgeon, and trust them to choose the right size and style implant for you.
Ultimately, your appearance is the most important outcome rather than the style or size of your implant!.
Augment mastopexy
I would recommend an augmentation mastopexy. A lift alone will not increase the volume of your breasts, especially along the upper poles. Implants alone will not lift your nipples to a desirable height, but combing the two procedures with an augmentation mastopexy should give you the best result.
I would not have the acne scarring treated at the same time as the breast surgery, but lasering or dermaroller are two possible options to improve the scars.
