Thank you for your question and photos. You are starting at a great place and can have a fantastic result. I would recommend seeing a BC plastic surgeon to discuss your goals and view his/her before and after photos. Depending on how much upper pole fullness you desire, a combination of a breast lift +/- small implant or fat grafting will get you there. Liposuction of your abdomen/flanks/back will give you a nice result as well. Best wishes!

In general, it is possible to have a breast lift and fat grafting at the time of implant removal. Unless there is something wrong with the capsule, total capsulectomy is not necessary. Make sure your surgeon is a board certified plastic surgeon and is comfortable with the procedure. Best wishes!

Thank you for your question and the photos. If you have saline implants, you can have your plastic surgeon deflate them prior to surgery so you can see what volume will be left. It is possible to remove the implants and do a lift at the same time. The entire capsule can be removed, but it is often not necessary. Yes, you can still get a breast lift, which will also reduce the size of your areolas. Your breast tissue will be repositioned to a more optimal location on your chest. This can be done with minimal tissue besides skin being removed. I would recommend then having implants placed at a second stage if you wish to have more volume. Best of luck!

Certainly a breast lift will give you better nipple and breast mound position with a moderate increase in upper pole fullness. This may be all that you desire. Unfortunately, it is impossible to answer without an in person examination and detailed review of your history. Many times it is possible to downsize the implant and fat graft the soft tissue envelope, but there are limitations to both. Thank you for the question. The LD flap can vary considerably in size based on technique and the patient's body habitus. In thinner patients, it is more often necessary to add an implant for volume and projection. In others, it can sometimes it can provide sufficient volume to reach the desired size, especially when coupled with fat grafting. This may require more than one procedure, depending on the anatomy and goals of the patient.

In general, switching from sub-pec to pre-pec position of the implant in reconstruction cases will eliminate the animation deformity. It could also, however, worsen your rippling as the implant will no longer have muscle coverage in the upper pole. It is difficult to say what cup size you will be with the final implant as based on the tissue expander size. Your body habitus, height/weight, implant selection, and adjuvant procedures (i.e. fat grafting), are factors to consider when discussing your goals with your surgeon.
