Breast implant sizing
The best implant size for you depend upon the base width of your breast and the quality of your skin. These are items that only your surgeon can assess. If your surgeon has suggested 410 cc implants based on yoru measurements and your goals, you should feel comfortable going ahead with this size.
I would suggest addressing your concerns about size with him or her directly. I’m not sure if this is available in the UK, but Inamed makes a sizing kit for patients via their Natrelle web site which many patients find very helpful in determining the size which they are hoping to achieve.
Differences between breast implants
All three of the brands you mentioned make high quality breast implants. They each have their own technology, but they are all safe and FDA approved. Some are a litter firmer and others are a little softer.
Prices are generally a little higher for the anatomic, or shaped, implants compared to round implants. Your body measurements and the look you are trying to achieve with surgery will also help you and your surgeon in selecting the best implant for you.
Most surgeons have a preference with respect to particular brands, but I wouldn’t say there are major advantages/disadvantages to each brand. I would recommend a consultation with a board certified plastic surgeon so you can feel the different implants in person.
The most natural looking and feeling breast implants are sub mammary textured silicone gel
I would not be so concerned about the high profile verses moderate plus but what implant placed where would look most natural. In my opinion the most natural looking and feeling breast augmentations are accomplished with peri areolar incisions with a textured silicone gel breast implant.
I tend to use high profile because most women want to have their breast stick out from their chest rather than bulge into their arm pit area. This method of breast enlargement often fools the patient’s OB/GYN.
The implants sit exactly where the surgeon puts them and follows the breast shape itself rather than sitting higher behind the muscle. I really discourage the use of Saline implants behind the breast because of so many folds and wrinkles.
Periareolar submuscular augmentation
Most breast implants are placed under the muscle, perhaps 80% or more, and the periareolar incision will work well for submuscular augmentation. In fact all approaches, around the nipple, under the breast, or underarm will work well with submuscular augmentation.
Breast feeding and implants
You certainly can breast feed with implants, but it is hard to say just like a woman without implants, whether they will be successful in supplying enough milk.
Breast implants for a very lean body
Actually your small breast and frame look like so many of our breast augmentation patients. The B-cup is a very reasonable goal, and if you have a moderate profile implant it will blend best with your breast, and submuscular is the way to go.
No upper pole fullness? 800HP is not for you
This re-discussion needs to happen with your doctor and you need to show him/her very specifically the “look” you like and what you don’t like. If you do NOT want upper pole fullness, an 800HP saline implant is NOT for you as it will look very full and unnatural, not to mention all the long term issues you can have with huge implants.
Time to re-evaluate the plan!.
Implant choices
Choosing the right implants for you requires a physical exam and a discussion of your goals. I can not predict what would be best for you without the above.
Saline vs Silicone
Usually saline implants are cheaper than silicone implants. You may want to verify the price you were quoted.
Breast implant surgery
Everyone heals a bit differently from surgery and the exact surgery may be different for different patients based upon pocket location and native breast size.
Family history of breast cancer is not a contraindication to breast augmentation.
A positive family history does not preclude a breast augmentation. The implants make mammograms more difficult but not less accurate. Interestingly enough, breast implants actually lower the incidence of breast cancer (not much but measurable).
We don’t know the reason for the influence and it is certainly no excuse to place implants but patients can be glad that they are not pushing the odds in the other direction.
Removal of the broken implants, the scar capsule and any loose silicone will make a big improvement
The implants made in the 70s were very thin walled and had very liquid silicone gel. Essentially all of them have failed and need to be removed with the over lying scar and any loose silicone or silicone granulomas.
The good news is that this should be relatively easy from your description of your breast. Along with this you could have a breast lift or new and much better implants or both. Interestingly my patients like you feel so much better after surgery because their breast are again soft and don’t hurt.
Lift first then implants later or fat grafts
I would first do a lift and the goal is to place you nipple in a better location and create a better shape. The next procedure 6 months later is to enlarge your breasts with an implant or with fat grafts if you want about a cup size increase.
Will Adding 50CC to my Implant Make a Difference?
Adding this amount will not make a difference. If you are not happy with the size and shape you may need a different implant.
Limitations of the periareolar lift
No one wants the scars from a breast lift, but they are very often a good trade-off in order to get the shape right. The periareolar lift gives a little bit of lift but not much, and it tends to flatten the breast.
Combining it with implants can help, and many plastic surgeons won’t do the periareolar lift (also called a Benelli lift) without implants. No one can give specific advice in your case without personally seeing you, but my guess is that the periareolar lift won’t quite give you the perky look even with large implants, which then no longer qualify as “perky”
Talk to more than one plastic surgeon and see what they say after examining you and listening to your concerns; hopefully they will have better news
