I think that a 265-275 cc implant is a relatively modest choice which would be consistent with your stated goals. I would also suggest that you focus on how natural and proportional you will look and less on cup size because there is so much variability among different manufacturers as to what constitutes a B or C cup size.

I would categorize your breasts as having a constricted lower pole rather than tuberous. In addition to the normal technique of breast augmentation, you will need release of connective tissue fibers which are causing this problem.

Breast implant size selection is not as simple as picking how big you want to be. The range of implant sizes that will fit your body are based on several factors using tissue based planning, and among the most important are the height and width of your breasts and the laxity of your skin (how much it will stretch). It appears from your photos that there is not much laxity to your skin and that will be a limiting factor in choosing a size. It is hard to imagine how a 550cc implant would fit...

You clearly need a breast lift and regardless of the technique used, you will have an "anchor" type scar. You do have a substantial amount of breast tissue so I would recommend a lift alone and breast implants at a later date. This approach will give you more flexibility regarding implant size without your surgeon having to worry about the tightness of the closure of the breast lift. There is also the possibility that you will be happy with your size and shape and will not need implants.

Base width of the breast is one of the important measurements used in selecting an implant size. Although it would be nice, you cannot just pick any size that you want. Implant sizes of 520 and 560 already exceed your base width and I believe your surgeon is right to refuse to go any bigger. However, that being said you should never go ahead with a cosmetic surgery procedure if you have any doubts that you will be happy with the outcome. Get another one or 2 consultations from board...

It may be the picture angle but the first photo looks like a different person that the next two. Looking at photos 2 and 3 I think you may have a mild amount of sagging but would still be a candidate for a breast augmentation without a lift. Of course there is no substitute for an in-person exam with a board certified plastic surgeon to confirm that this would be the best approach. Good luck.

There is evidence that capsular contractures are related to chronic inflammation but I am not aware of a higher risk in the presence of MS. Regarding the risk of a flare-up after surgery, that is a question best answered by the neurologist taking care of your MS. Good luck.

As you can see, opinions differ but both approaches are perfectly acceptable. I prefer under the muscle because there is more long-term support of the weight of the implant, contractures of the implant capsule and mammagrams are easier. If you truly have a depression of the bone between your ribs, leaving a layer of muscle intact along the inner boarder of each breast would counteract any tendency of the implants to drift towards the middle. Complications following breast augmentation are very uncommon but can and do occur. Some complications may require going back to the operating room or even hospitalization. So without knowing what the clause you are referring to actually said, if it is along the lines of what I have described then I would say yes this is normal. If you are still unsure discuss it with your plastic surgeon.

Second, it creates an abnormal tunnel between the armpit and the implant pocket with a tendency for the implant to move back up this tunnel....

The first order of business is to get the tooth abscess treated and healed because you do not want an active infection to potentially contaminate the implants on the day of surgery. In the future, you and everyone else still needs regular dental care. There is no clear consensus (or strong evidence either way) regarding the use of preventative antibiotics before having your teeth cleaned and examined. Discuss it with your plastic surgeon and dentist.

In your particular case, if you are getting a breast reduction by definition your breasts are too large and heavy. Otherwise, you would be getting a lift alone. There would not seem to be much point in making your breasts smaller and lighter just to make them bigger and heavier, especially if you are having shoulder pain.

The photos are somewhat limited but based on what I can see, it appears that the left breast sags too much to manage with implants alone. Also if you want upper pole fullness you will need implants as well. So I would recommend a vertical breast lift with implants, which will leave you with a "lollipop" type scar. Beast of luck.
