Plastic surgery is all about tradeoffs – as far as cosmetic breast surgery goes, the decision to lift or not has to do with how comfortable the woman is with the scarring that will result from a breast lift.

You do have some breast ptosis, and asymmetry in your nipple/areola position and size. So objectively, I agree with what’s been said that a lift will improve your appearance.

However, YOU have to be comfortable with the tradeoffs of a mastopexy procedure – increased scars, risks, cost etc.

I have had some extremely happy patients who ‘needed’ a breast lift undergo breast augmentation only. This was only after thorough counseling about the limitations, and the look they can expect. I show before/afters of patients who have made either choice, so make sure you look at some of these before deciding.

It is true, however, that going with larger implants simply for the ‘lift’ effect in order to avoid a formal lift is usually a mistake in the long term. I hope.

It should really be your surgeon helping you choose a size. Taller people do need bigger implants. But if you are unsure you should consult another surgeon in person.

650 cc breast implants

It is very rare not to need a lift after massive weight loss. However, if that is the case, a 650 implant will be quite large on you and may possibly have the undesirable effect of making you look heavy again.

Check with some other surgeons for an exam and recommendation.
Breast implants without breast lift?

If you were to place implants alone, I fear you would suffer with a double bubble deformity as your natural breast tissue has undergone too much stretching and sagging from pregnancy and breast feeding.

Your natural breast tissue would “fall off” the implant and create a Snoopy nose appearance. The weight of the implant would also exacerbate your breast sag. In order to best enhance your breasts, a lift would be needed to help elevate your breast tissue and nipple-areola into a higher, more youthful position while allowing resizing of your areola.

A breast lift can be combined with implant placement if you are interested in increasing your overall breast size.

Need for lift (mastopexy) with a breast augmentation.

the reason the breast sags is that the “skin covering” is proportional larger than the breast tissue. the majority of your breast tissue is below where the breast mound should anatomically be located.
if you refill the volume only(breast implant)… the nipple areolar complex (NAC) will rotate over the top of the implant like a waterfall resulting in a full breast mound but unfortunately a downward rotation and location of the NAC.

the only method known to create a aesthestically acceptable breast will require trading incisions/scars on the breast for a pleasing contour and relocating the NAC.

depending on your desires (shape and size) the scar may vary from: A) just around the nipple (doughnut) B) to a added vertical incision (lollipop) C) to include a horizontal incsion in the breast crease (anchor)

while breast incisions/scars are required.

there are ways to modulate/improve the appearance of the scarring process. Please make a appt. with experienced Plastic Surgeon who is Certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (denoting by membership as having met additional criteria and a focus on Cosmetic Plastic Surgery).
Mastopexy-augmentation

If you are after a full breast look, and you are already starting off with a fair amount of your own breast tissue, I do think that you could start by just adding the breast implant.

Now, you may not be satisfied with the nipple position (it may be too low), but you could always stage the two procedures and only have the lift component later if you need/want it.

That way, you are also dividing the costs a bit.

Now is o.k for breast augmentation with the following precautions

In my practice I like to wait until the milk production completely stops ,however nine months out from breast feeding cessation is a long time. I would do the following:

1- Discuss this with your board certified plastic surgeon. There are medications to help stop the milk production. Your Family physician or OB/Gyn would also do an assessment to exclude any other causes.
2- Technically I would choose an approach that does not require breast dissection (inframmary or axillary) to minimize potential contamination of the implant.

It is the small risk of infection that we usually worry about.

Lift and implants

To try to give you the best result possible, I think that a lift would be the best thing to add to an augmentation for your.

Do I need a lift or an augmentation

Be aware that implants won’t raise the breasts up, and a lift won’t enlarge the breasts. Without seeing photos of you I would believe that you need a lift as well as using implants a proper evaluation should be performed by your local Plastic Surgeon.

I would feel that at that time suitable recommendations for treatment would be made at that time..

Avoid peri-areolar incision

There is very little difference between a 425 and 450cc implant. This question is better answered in person. I, personally do not recommend the peri-areolar incision. The nipple is the visual focal point of the breast so why would you want a scar near there? The chance of losing nipple sensation is higher.

Statistically, infection and capsular contracture rates are higher as well. My # 1 choice is transaxillary with endoscopic guidance or the inframammary fold.

No such thing as “Free” breast implants

As I am sure you are aware, nothing in life is “free”. There is always a price to pay. I would implore you to think about the long-term ramifications of allowing unknown persons to finance your surgery.

I have never had a patient inquire about paying for their surgery through this method. Although how you pay for surgery is a personal choice, I will tell you that I do not think I would be comfortable operating on a patient who would use this method.

it’s all about the integrity and I would never feel good about myself as a surgeon or a person who condones this by performing a breast augmentation financed by this “free” site. Please consider what you are doing.

Continue working hard and saving and I can assure you that when your time comes, you will be very proud of having worked so hard for something that your obviously want so badly. I hope you will think about everything that my colleagues and I have said.

Size choice

Your height and weight don’t have much to do with implant size selection. Your Plastic Surgeon would have measured your chest/breasts carefully and these measurements along with your existing tissue/anatomy determine which implants fit you.

You would have had a small selection of options ranging in size- usually increasing in volume/projection, but the width of all your implant options would be similar, fitted to your breast base width. If your Plastic Surgeon is telling you that the 550 implants fit you and you liked them when you tried them on, then don’t worry about the number.

Everyone’s body/measurements are different. If you are not certain about your choice, ask if you can try on the implant sizers again. My patients find this the most helpful for deciding.

Looking at this picture, do you think a BA will be suitable or will I need a lift

I don’t think a lift will be necessary with a change as large as what you want. On the other hand I think that what you want is excessive and will result in overly large unattractive breasts.

Do i need a lift?

I feel that you definitely need a lift as well as an augmentation. Many patients are frightened by the scarring but overwhelmingly they feel that the pretty shape and size they obtain is well worth it.

Beware of New and Improved

The current generation of cohesive gel breast implants have completed 10 year studies with excellent safety outcomes (https://www.natrelle.ca/silicone_safety.asp). There is no great push to improve on the current implants. This should give you good confidence that the current product is a good one.

You should always be concerned when you see a product that is ‘new and improved’, because it generally means there was a problem with the old product, and there would be insufficient experience with the new and improved product to be confident the improvements will hold up with time.

Recent attempts to improve on breast implants (e.g. soy-filled implants) were not successful, even though the product seemed to be an improvement over silicone filled implants.

Even if a fantastic new implant became available tomorrow, you would still do better to use the current implants and wait for the new implants to prove themselves safe over the next 10 years.
