No injectables, no lip lift, no problem. It sounds like you may be a candidate for fat grafting to your lips for more volume. It's a permanent solution, whereas fillers typically last 6 months, and it wouldn't change your philtrum distance.

If you want more volume and fullness to your lips, fat transfer makes the most sense vs a lip lift. A lip lift is a procedure specifically to shorten philtrums (with the added benefit of revealing more of the top lip) so yes, it's most appropriate for people with long philtrums.

A second round of fat grafting can be done at least 6 weeks after the first.

Do I need a brow lift/anchor blepharoplasty?

Upper eyelid surgery with ptosis repair is appropriate in this case. It's a relatively quick and easy procedure that can be done under local anesthesia. It would effectively address the feeling of heaviness and unevenness that you experience.
Can you combine hydroquinone with Retin-A or do you have to apply them separately?

Use hydroquinone on its own in the AM. Then use both hydroquinone and Retin-A at night - doesn't matter which one goes first. You can mix it if you want. If you experience redness, sensitivity, or irritation, use the Retin-A less often.

Liposuction would be helpful in removing excess fat, but you will likely be left with loose skin. A tummy tuck is more appropriate in removing the skin as well as fat. Some of your stretch marks will be removed in this process and what's remaining will be flattened but not completely gone.

You are beautiful and do not necessarily need any surgical enhancements - unless you want to. Your concerns are valid and can be addressed in a variety of ways (surgeries included). However, before considering permanent surgeries, you may want to consider injectables like nose, chin, or jaw filler.

As long as it's not actively infected, you can absolutely get a BBL. That area could be avoided entirely and the necessary incisions needed for the operation can be made elsewhere.

No shade to these ladies, some of them just look a little anatomically impossible. However, it's totally realistic if they had a lot of fat grafting or implants. Many places have limits on how much fat can be removed in single operation (I have a 5 liter limit at my practice) for safety. Depending on their starting point, these ladies likely have had multiple operations.

A tummy tuck and liposuction alone will help flatten out your stomach and contour your body. Your waist will be smaller and by comparison, you may feel that your buttocks is more prominent. However, the only way to give you more volume and projection to your buttocks is with fat injections or implants.

If your cardiologist has already cleared you for elective surgery, you have a good chance of moving forward with the procedure. It's typical to require blood work as baseline and they may very well be requesting an EKG to establish a baseline - it doesn't always mean that an abnormal EKG reading would disqualify you. Ultimately, it depends on your surgeon to make the call.

The main issue here is that with the excess skin, liposuction alone was not the appropriate first procedure. Your doctor should have prepared you for that, if they didn’t. The only way to correct this now is with a full necklift with incisions behind the ears. Best of luck.

While 5 weeks is still relatively early in the healing process, it should be enough time to see an hourglass figure. For an hourglass figure to occur, you need at least two things: 1) aggressive lipo to the love handles to narrow the waist and 2) fat injections to the hip dip area (outer thighs) to widen the hips.
