Breast Cancer and Breast Implants

The risk of developing breast cancer will not be affected by breast implants according to the scientific literature to date.

Mammograms will miss a small percentage of breast tissue for future screening but this is more of a theoretical concern and for that reason I would recommend a submuscular placement leaving more breast tissue available for mammographic examination.

High Profile Implants Better for Deflated Breasts?

It appears that you have thin tissues and some sagging, with your nipple position probably below the level of the fold and at the lower edge of the breasts visually.

This makes me think that some type of a “lift”, possibly just a “nipple lift’, may help give you optimal correction. Combining a lift with augmentation requires a greater level of surgical skill and experience.

Restoring volume with high profile implants may help fill your tissue envelope better, depending on the match with your breast dimensions, but you may want to consider gel implants. The thinner the tissues, the more important it is to consider gels for a more natural look and feel, and less likelihood of visible or palpable rippling.

Lift

YES you would benefit from a lift and if you wish to be augmented then in implant of a volume you like on your chest would be chosen. So see some experts in your area and become informed.

One relatively easy way for you, to determine the location of your nipple in relation to your chest breast juncture is, placing a pencil under your breast, if the nipple is below the pencil, pretty much you will need a lift.
Be sure to seek the help of a plastic surgeon member of the American Society for Aesthetic Plastic Surgery .

Can steam rooms damage breast implants? Does the heat compromise the shell and would cause to rupture sooner?

You will not cause any damage to your implants from steam or any other type of heat source. Your skin will burn before you damage the implants.

Dropping & Settling of Breast Implants Post-operatively.

At three weeks post-op, patients are generally a long way from the final result. It is common for implants to be high and tight at this point. The implants will settle into place over the course of 2-4 months; and they mat settle at different rates.

Implant displacement exercises are helpful in reducing the risk for capsular contracture, and enhance the settling of the implant. After implants settle into place there may still be other changes that occur as the inferior pole stretches further under the stress of the implant.
Therefore, the final results may not be seen until six months to one year post-operatively. Your pictures show that the left implant is in good position at this point. The right implant, in comparison, is higher and tighter with less inferior pole projection.

I have some concerns that this implant may not drop as anticipated, but would not panic at this time. This implant still has plenty of time to settle in and displacement exercise should be continued at three times daily; massaging the upper implant to facilitate downward movement may also be helpful.

If the implant still remains high at three months, I would suggest you discuss this with your surgeon. I don’t understand what type of indentation you are seeing on the right, and I cannot recognize this on the photograph.
I think it is rather early to see implant wrinkling and/or rippling, especially in patients with greater soft tissue coverage.

Lift with implant is best plan

Having performed many implant and lift combination surgeries I can tell you you won’t be happy without both. The weight loss you underwent resulted in lost volume, however the previous larger volume you once had resulted in stretch and sagging of the breast.

Both issues, volume and sagging, must be addressed. If you do just one part of the surgery, you will likely be unhappy and ultimately procede on to the other surgery quickly. Find a surgeon very comfortable doing both procedures together.

I am afraid that I have to agree with all the other answers. Your concerns are well founded and I agree you are correct in being concerned that you will get a poor result from implants alone.
The problem is your have a loose skin envelope and your nipple looks to be sitting well below your fold. An implant alone is unlikely to provide enough lift to give you an aesthetic breast shape.

I do however understand your concerns about scarring which are common concerns. What you need to put into perspective is that good quality scars (which are largely hidden) with a great breast shape, will look far better than a small scar with a bad breast shape.

I am interested in your choice of implant size – I assume you are choosing that size implant in the hope that it will give you some lift. it would seem to be quite a large implant for your frame size and breast foot print.

Combination mastopexy and augmentation can be a challenging procedure. The revision rates and complications are higher than for either augmentation or mastopexy alone, and you may be better off having two separate procedures.

You should discuss all of your options along with your aesthetic goals with your plastic surgeon.

You need to make sure that he is supervise

All of us gone through the resident clinic as part of our training and done our first breast augmentation under supervision. The guide line for the resident cases are supervision by board certified plastic surgeon.

I train plastic surgery residents and they never do surgery without direct supervision. If you are not comfortable with your surgeon, you should cancel your surgery. If you do not trust your surgeon before surgery, you will never be comfortable with the results.

Doctors and patients should choose the breast implant together

By working closely with a patient and spending at least an hour personally with them, the doctor and patient can mutually agree on what will be the best implant for you. A doctor who puts the whole decision on your shoulders is a huge red flag from my standpoint.

It is safe, and actually encouraged, for you to wear an underwire bra after augmentation

For the first few months following breast augmentation, most patients will notice that their implants are too high. During this time, it is best not to provide substantial support to the breasts to encourage the implants to eventually make their way to an appropriate position within the pocket.

At that point, around 2 months following surgery, it is recommended that you wear an underwire bra to keep the implants from sliding any further down. In addition, over time it is best to wear a supportive bra so that the internal ligaments (“Coopers Ligaments”) have as little tension on them as possible.
When they become stretched out, your breasts will eventually droop and you will require a lift.

Less pain with breast implants the second time around (remove and replace only)

In my experience, the vast majority of pain with a revision breast augmentation is a fraction of the intiial procedure and I would estimate probably less than 30% (based on pain medication use). This is true for straightforward breast implant removal and replacement and does not include lifts, revisions, capsulorrrhapy, capsulopexy, capsulotomy, size change, etc.
