Breast implants for saggy breasts
There are several options that incorporate breast implants to help sagging breasts. There is no best implant, but bigger implants (to a point) work better than smaller ones. Placing the implant above the muscle may give more of a lifting effect, but only in patients who have enough breast tissue.
Your areolas appear a little low and large… you may be a candidate for augmentation with a periareolar lift. My best advice to you is to seek consultation with a board-certified plastic surgeon with experience in augmeantation/lift.
You should be fully examined so that the best surgical plan can be developed.
Alternatives to Breast Augmentation/Implants
Unfortunately, there are no safe, scientifically proven, and permanent non-surgical alternatives to increase the size of your breasts aside from undergoing a breast augmentation procedure.
While some websites and magazines will advertise breast enhancement creams and pills, I would caution against these as they have not been proven to be either safe or effective alternatives and would simply be a waste of your money.
I would highly recommend consulting with a board certified plastic surgeon who specializes in breast augmentation procedures. At the time of your consultations, all of your concerns will be addressed and I believe you will find that undergoing a breast augmentation procedure is a safe, quick and effective method of breast enhancement with very low risks of complication.
Are you replacing the implants or just removing them. either way it seems excessive. a simple removal can take as little as 30-45 min. replacement a little longer unless you have extensine capsule work which needs to be done
Breast lift
It appears that you need a full breast lift with implants to achieve your desired result.
What is your goal?
If you want your breasts to be very large and not natural then this may be the size for you. Your base width may be 12 cm and a Moderate profile implant of about 330 cc would be natural and you would be a D cup.
The extra high profile you and your doctor have chosen is much lager and will likely make you an F cup. Please look at before and after pictures to try to get an idea of what size is best for you
Incision
I would go on several consults and get the opinion of experts in your area – though certainly the periareola approach is very commonly used and I have found it to be very good in the right patient .
Define your goals
If you don’t want to be any bigger, implants are a poor choice. The size implants your surgeon has suggested are too small to give you any sort of upper pole fullness. So you are stuck.
If you wanted to be bigger and have upper pole fullness than an appropriate sized implant would do that.
Size is an individual decision with the input of your surgeon after a complete examination and measurements. It would appear that you would also benefit from a lift. Go on consults with experts in your area and get their opinion after a full examination.
I like when patients come back for a second consult. Gives us a chance to make sure everything will go smooth. The more information for you the better. Go to a board certified plastic surgeon.
Running after implants.
I advise my patients to avoid any activity that will raise the heart rate or blood pressure for two full weeks after surgery. This included walking, running, biking, lifting, tugging, or pulling over 20-30 lbs.
At the three week point you may resume lower body activities such as brisk walking, lunges, and squats. At four weeks after surgery you should be fine to resume your regular exercise regimen. Always wear a good supportive bra while you run.
No..present implants for cosmetic augs probably better
For cosmetic augs, the presently available implants will not likely be surpassed by the new ones for softness and natural look. I don’t see any reason to wait for “new” technology at this time.
Tuberous breast correction
I would agree that you appear to have a tuberous breast deformity. This is evidenced by the constricted appearance of the breasts, the apparent herniation of breast tissue into the areolas, and the elevated inframammary folds.
Correction of this deformity is typical performed by releasing the internal constriction (cutting internal bands which cause the herniation and conical shape), lowering the breast fold, and increasing volume with an implant. Volume can also be improved with a latissimus flap or autologous fat injections as well.
Simply releasing the constriction often leads to flattening of the breasts which may not give you an acceptable appearance. This is not a “straightforward” surgery, and I recommend a consultation with a plastic surgeon who has experience treating tubular breasts.

370cc implants will fit very differently on each patient so you will need a proper in-person assessment of your breast dimensions to determine if they would be appropriate for you.
