I think that you would get away with only placing a breast implant. Your breasts will be lufted to some extent by placing a big enough implant. Since you want to go from B to D cup we can assume you would need implants of the size 400cc +- (360-410ml). If an additional lift would be needed I am confident that a periareolar excision of excess skin would be sufficient. Theoretically you can have a breast augmentation at this point. Important would be that you wouldn't use a periareolar incision to insert the implant. To place the implant the surgeon will also have to cut through your breast tissue which will heal with scar tissue. Please don't panic. 4 days post-op is extremely short after your surgery and your breasts should be swollen most at this point. I am sure you surgeon put some consideration in the choice of your implant size. I can understand your concern. Just after one week the wound is still quite fresh and a post-op bleeding can be caused by sudden trauma. If you don't experience an increasing swelling of your breast you can calm down and relax. If you experience increasing swelling please contact your surgeon so he can assess you.
