Breast implant placement

Given how thin you are and the lack of breast tissue to start with, my recommendation is for placement under the muscle. This will produce a more natural feel and appearance and reduces the risk of capsular contracture. For sub pectoral implantation, I use smooth implants as there is no benefit to the textured implants except in the rare patient who is a good candidate for placement above the muscle.

Large implants

650cc implants will give a different result on every patient. I have put 800cc implants in that did not make the patient too big. But that was a very large frame patient. You appear to have a small frame.

650cc implants will be very large for you. Trying on implants does not work very well. I would recommend that you see your surgeon again, look at photos of his patients and discuss the look you want.

Forget about having a certain size of implant, it is the result you are after!

Periareolar incision?

Periareolar incisions were in vogue in the early 90’s when saline implants were used exclusively for augmentation. Currently silicone is also used (most frequently) and thus an inframammary or transaxillary incision is preferred. If you have an unsightly scar after a periareolar incision it is very visible.

Each person must decide what the best option is for them though. Using a Keller funnel and a small implant, a periareolar approach can be performed. Discuss with your surgeon.

From what you have said, I would recommend silicone implant placed in front of the muscle. There may be a little higher risk of capsular contracture, but it will fit your lifestyle better.
Implant type

I recommend that you get a professional opinion from a board certified Plastic Surgeon in person. You may have many options available to you regarding implant type/shape/placement and your Plastic Surgeon will be able to explain the pros and cons of each.

My patients, including those under 22, tend to prefer silicone cohesive gel implants for their natural look and feel.

BREAST ENHANCEMENT

You pose a question that many women have asked. Unfortunately there are no natural remedies, herbal products, or exercise regimens that will significantly increase breast size. Exercises that target the upper body and pectoral muscles enlarge the muscles of the body which can cause an increase in breast size but once the exercises are stopped the breasts will return back to normal.
In my opinion the only effective way to increase breast size is through breast augmentation. The breast augmentation procedure is minimally invasive and typically lasts around 2 hours. The procedure can be done with moderate sedation and in some cases utilizing only local anesthetics.

The recovery time for breast augmentation is relatively minimal. Typically pain medication is only needed for the first few days if at all. In my practice I routinely have patients who have surgery on Thursday and return to non-strenuous work on Monday.

I recommend you find a board certified plastic surgeon utilizing RealSelf. During the consultation process be sure to discuss your fears and concerns.

Breast implant after reduction

You would need to be seen in exam in order to determine the correct size for you. Make sure you are going to a board certified plastic surgeon. I would suggest bringing photos of other women who have a similar breast shape so that he/she has an idea of what you wish to achieve.
He / She should have you try on sizers to get the best size for your desired cup.

Breast Implant ruptures

Vigorous acitivity or compression from mammograms should not cause a rupture of the implant.

But, a rupture can happen with extremely strong force like car accident that causes rib fractures. The implants are pretty strong products.

Breast Augmentation after Breast Feeding

3 months is a general guideline for breast augmentation after breast feeding. However, some patients still have evidence of lactation 3 months after the cessation of breast feeding. In these cases, it’s best to wait until lactation has completely stopped and the breasts have reestablished a stable size.

Bra shopping after Breast Augmentation?

I’m glad to hear that you are please with the results. I would suggest that you avoid spending a lot of money on expensive bras for an additional couple of months.

Your breasts will be changing during this time period.

Silicone Implant Rupture and Exchange
Silicone does not pose any real health risk. Numerous studies over the past 20 years, including several of my own, have clearly demonstrated this. So, there is no urgent need for implant removal. However, silicone migration to the lymph nodes can cause inflammation and scarring which could become a source of discomfort.

Implant exchange, following MRI-proven rupture, can easily be performed.

Breast augmentation will create pleasing proportion

You would have a nice improvement in your proportion if you had breast enlargement. There would be a much better balance between your upper and lower half.

In our office, if a young woman is mature, has realistic expectations and can fully appreciate the pros and cons of breast implants, then she is a candidate for surgery.

Breast augmentation is a very good option; the question is how you feel about the pros and cons of breast implants.
Some doctors are performing breast augmentation using the patient’s own fat, which I do not recommend at this time. Look for a board certified plastic surgeon in area for a consultation. Your plastic surgeon will be able to help guide your decision regarding whether or not to havce breast augmentation.

Saline vs silicone

Your concern is a common one when it comes to choosing between silicone and saline implants. However in general, patients enjoy a more natural-feeling and -looking result with the silicone implant. To learn more about the risks associated with silicone implants, talk to your surgeon who can address your personal concerns.

Breast Implants Can Feel Natural

While some plastic surgeons vary on their preference on using silicone gel or saline implants, I believe that most would agree that the silicone gel implants, generally speaking, tend to feel much more natural than saline implants.
How soft and natural your implants will be can also be attributed to factors such as, amount of native breast tissue, placement of implants (sub-pectoral or below the chest muscle is overwhelmingly preferred), size of implant, profile of implant and surgeon expertise.

Hardening of implants is often attributed to capsular contracture or scar tissue formation, and can happen with both silicone or saline implants.
