I determine migration from the patient's position on their back and if the implants go off to the side of the chest. Several lifestyle habits can contribute to lateral migration that I steer my patients away from. 1. isolated pectoral exercises pound your implants out laterally, which can contribute to the lateral breakdown of the implant pocket; 2—being a tummy sleeper. 3. a genetic disposition to weak tissue.

The thing about plastic surgery is that good surgeons will tell you that you can't always have everything you want.....for your safety. Your surgeon is probably trying to keep you safe by respecting your chest measurements, skin quality, etc. Suppose a surgeon puts in implants that are bigger than your dimensions or puts too much stretch on the skin. Your skin will fold and wrinkle when you bend, like paper, cloth, or anything else. You have to have enough skin to stand straight, but it's excess when you bend so you will see folds. A tummy tuck might be in order if your skin is hanging when standing up straight, but no surgical intervention is indicated otherwise. Best Wishes!

Your implants have dropped too far down and the skin has stretched. That makes your nipples at the top of your implants instead of in the middle. The lower pole breast skin can be shortened with a "horizontal mastopexy," but you may need the skin reinforced with some type of "mesh." I prefer Strattice. If the implants are above the muscle, I recommend placing them below the muscle and using the Strattice to reinforce that skin and fold, like an internal bra. Best of luck...

If you do sub-muscular breast augmentation only, the result will look like your breast is hanging off of your implant. If you do sub-glandular breast augmentation, the result will look like larger hanging breasts and not provide upper pole fullness. Your best bet (in my opinion), is to complete childbearing, breastfeeding and then get your lift with an implant. Multiple breast surgeries increase your risks of losing sensation, scar tissue and lots of money.

I don't necessarily think you have tubular breast deformity because the lower crease beneath your breast is expanded, not pinched, and your breast does not "topple" over the fold. A donut mastopexy would lift your nipple slightly and downsize your areola. Nothing will provide additional volume and/or firmness except an implant. I think you do not need a full anchor scar breast lift. Best wishes!

My recommendation depends on how far post-op you are. This would not be an unusual scenario a couple of weeks post-op, but it would be unusual a year post-op. I recommend giving your recovery 9-12 months and re-assessing your result. Recovery is an evolution. Best Wishes.

You picked your surgeon for his/her expertise and results (hopefully). Please listen to the instructions given by your surgeon and follow those. Everyone has different opinions and reasons why they like what they like for their patients.

If your scar was in the fold beneath your breast and is now higher up on the breast, you might be "bottoming out" but I don't think it's that because your implants don't look like they have dropped into the bottom of your breast pocket. Your implants may not have "dropped in" properly or you could have a contracture. Either way, You're right, they don't look like they should. I recommend seeking another "in person" opinion from a Board Certified plastic surgeon.

I am a big fan of natural-looking nipple position and no scars on the front of the breast unless it will create a result that wouldn't make most people happy. I am guessing by your photo that you will need at least a donut lift to raise the nipple-areola complex more in the middle of the implants. Of course, every plastic surgeon has a different opinion on this, so meet with a couple of different doctors and get opinions. Best wishes to you.
