In general, the pre-periosteal mid-face lift is associated with lower level of facial swelling and faster resolution of the swelling compared with the sub-periosteal lift.

In view of the malar oedema there is a theoretical benefit for the super-periosteal or pre-periosteal approach, but as you have highlighted, there is no structured study to compare them .

The lift effect provided by the two approaches is reported as similar.

In general, deep plane procedures for the face and neck can be revised but sometimes they just need adjustments.

There seems to be still some laxity under the chin and often the best vector to reduce it in valves scars in from of the temple hairline. There seems to be also some volume under the chin that could be addressed together with tightening and repeat platysma plasty.

There are several statements here together. People can look fine and still be asymmetric because facial asymmetry affects all of us.

One of the aspects of facial asymmetry is that the position of the eyes and the eyebrows is not the same on the right and left sides.

In a lot of instances, it is worth waiting a little bit before doing surgery as the skin will be able to adapt to some degree and you will make a better decision.

Fat transfer on its own may create a disproportion with the rest of your features and it is more likely that the facelift would generate an alignment between your facial features…

Facelift surgery has a tendency to improve these lines but doesn’t necessarily get rid of them.

An important consideration is to use sunblock regularly to minimise the effect that the ultraviolet light has on the elastic collagen fibres.

For the problems already in place, small amounts of fat transfer can help these lines and make them less visible. They will be less noticeable anyway through the facelift when the face is relaxed…

The distribution of the volume is somewhat unusual and the photographs do not seem to show it equally well depending on the lighting.

You have the option to try and get more clarity regarding the nature of the lump using ultrasound. Soft tissue high resolution ultrasound can produce quite valuable information leading to a better decision.
