Transareolar incision more than breast implant type

You may need a breast lift, as breast implants do not lift the breast. I have found that placing breast implants through a transareolar incision during breast augmentation is a better method for breasts that have sagged.

Will Breast Implants Lift my Breasts?

Breast implants are never a good substitute for patients who need a breast lift…. In the sub-glandular position (above the muscle), or with a well-done dual plane technique, an implant may camouflage breast sagging a bit, but possibly at the expense of longevity and the overall outcome.

If your goal for shape requires a re-shaping of your breasts, I would strongly recommend you find a surgeon who can perform a breast lift you would be happy with. When done well, breast lit scars usually fade remarkably well, particularly when scar management techniques are brought to bear.

It is never a good idea to choose an operation because it is the one you feel you can afford, because it is the one that fits into your schedule, or because it will give you the scar you think you want.

Rather, you should have the operation that is most likely to give you the OUTCOME you want. The only thing worse than being unhappy with your appearance is spending a lot of money, having surgery, and STILL being unhappy with your appearance.

Removal or replacement of breast implants

Many patients choose to remove their saline implants and replace them with silicone gel implants. I would recommend going through a peri-areolar incision instead of the armpit. There are less complications and risks going through an areolar incision.

Also, it decreases the risk of implant malposition.

Lift without lift scars

No one want scars on their breasts. Whether you need a lift or not is less about the surgeon and more about your anatomy. Implants are very inefficient lifters and the added weight causes the breasts to droop over time.

Think of it this way. You are already drooping. Do you think that adding an implant that adds weight to your breast will lessen the droop.

The trick is to find that surgeon that will do the best job for you artistically and have the most minimal scarring.

Breast enlargement cream?

I am not aware of any topical creams that can be applied to permanently increase the size of your breasts. If there were a product like this on the market that actually worked then everyone would know about it and we would be doing very few surgeries to increase breast size.

I agree with you that if you gain weight you will likely see an increase in your breast size, but this is usually not an acceptable way for women to increase the size of their breasts.

I would approach products like this with caution and realize that there is no scientific evidence for the results they claim.

Do I have a uniboob?

Based on your pictures, all looks well, and it does not appear that you have synmastia. Allow your results a few more months to fully heal and mature as it looks like you will have a very nice result.

The capsule around the implant can harden

Silicone and saline breast implants do not change over time. Our bodies create a “capsule” around all implants since they are foreign substances.

These capsules, in some patients, can harden over time, and that is called “capsular contracture.”

There are different levels of capsular contracture. Some patients never get it, others get a very mild version when you can feel a little firmness but cannot see anything, and yet others can get a more severe version where you can see and feel a very hard capsule that distorts the shape of the breast.

Many factors can contribute to why you get the contracture: infection, bleeding, etc. It can happen in one breast and not the other one. It is poorly understood, but common in about 10-15% of breast implant patients in most studies.
If you have not had a breast augmentation procedure yet, you should make sure to ask your plastic surgeon during your consultation about capsular contracture. If you have already had the procedure, see your surgeon again to go over some treatment options.

Rippling, small implant

Rippling is due to capsular contracture or underfilled implant. It can be visible if the overlying breast tissue is thin. If it is a small implant with good overlying breast tissue chances of rippling is reduced.

Implant Profile for Upper Pole Fullness

A high profile implant is likely what you would need to achieve as much upper pole as the photos have illustrated. A moderate plus or moderate implant will have a much more natural slope in the upper pole and would not have the excessive roundness that you desire.

What Exactly Is A Gummy Bear Implant?

The term “gummy bear” implant is widely used but poorly understood. Initially, this term was coined to describe form stable or anatomic type implants which held a tear drop shape.

However, in the last several years, this term is being applied to cohesive silicone implants which are round and not form stable.

These implants contain a cohesive silicone which resists extravasation outside the implant capsule. Allergan, Mentor, and Sientra make both form stable and cohesive implants which fall under the general description of a “gummy bear” implant.

Earlier replacement is better than later

Since the fluid from your saline implants should be harmlessly absorbed by the body in the case of a leak or rupture, breast implant replacement isn’t necessarily an urgent issue. However, it’s certainly better to have the matter addressed sooner rather than later.
The appearance of one deflated breast can feel somewhat disconcerting, plus the process of implant removal and replacement becomes more complex after the scar tissue changes to accommodate the now deflated implant. Be sure your surgeon has a good deal of expertise in breast implant revision surgery before scheduling an appointment.

Replace only if necessary

Breast implants – saline or silicone – do NOT need replacement every ten years. The shells of either do break and they are not lifetime devices. Therefore, at some point, you should expect to have to replace leaking implants.

The ten year timeline comes up because that is about the average lifespan for a typical implant. However, that is not to say some don’t last significantly longer – I have seen saline last up to 26yrs and silicone about 30yrs.
I have also seen much less time than 10yrs also. Average numbers do not mean anything about your specific implant. Also, since leakage of either saline or silicone is not harmful, there is no need to replace implants unless there is proven breakage of the shell.

With saline it is easy. The implant will simply deflate and become smaller than the opposite side. With silicone there will be no change in size and you may not have any symptoms at all and you need an MRI to detect any leakage or break in the shell of the implant.

Basic rule is to maintain a relationship with your surgeon for the long term and follow-up as you have questions or concerns.

Hesitation about surgery

With regards to your relationship with your surgeon, most people would look for a surgeon who they trust and feel comfortable with professionally and personally. You will have to decide how you proceed with that issue.
With regards to the surgical plan itself, you have significant ptosis and you need a mastopexy. I would not advise large subglandular implants in anyone with such a degree of ptosis since it is nearly certain that you will end up with a deformity, not an attractive breast.

The weight of the implant will stretch and damage the tissues progressively over time and the inevitable breast lift will be much more likely to result in healing problems due to tissue thinning and the greater disruption in blood supply that occurs with subglandular implant placement.

If your surgeon is trying to convince you to have a lift it is not because he is trying to upsell you, it is because you need a breast lift to have an attractive result.

Too Much Ptosis for Implants

An augmentation with implants alone will not achieve your desired look. You will likely have a “snoopy deformity” appearance. This is where your breast tissue looks like it is falling off your implant like a waterfall.

You will require a lift with your augmentation to elevate your nipples and redrape the skin around your implants.

I prefer submuscular placement

As you can see, there are many opinions about the best placement of an implant. Certainly, the patient’s body habitus plays a role, but really only if you are looking for a reason to go above the muscle.

Remember, you can generally always go below the muscle in a first time augmentation patient. The only implant I would ever consider using above the muscle is gel because the salines are too wrinkly in that position, or are pumped up too full and hard to compensate for the ripples.
But I would still put the gel or saline under the muscle anyway. The look over time is less edgy, the capsular contracture rates seem to be lower, the implants are less easily felt and mainly, mammography radiologists say they always prefer to image a breast when the implants are separated from the breast gland by the pectoralis muscle.
