Breast Augmentation and Age
Breast augmentation can be performed regardless of age. The surgery is safe and effective.
Determinants of breast augmentation are based on general health, anatomy and expectations of the patients.
Lift or not
In my opinion you absolutely need a lift, wait until your finances allow you to do the entire surgery because your result won’t be good without a lift .
Implants
being plus size will not limit your choices for implants. once you decide to do the surgery, you will need to decide on size of the implant. your surgeon will help you do this.
Deflated Breast Implant
A breast implant is not going to puncture your lung if it deflates. Your ribcage will protect your vital organs.
However it is probably in your best interest to get a new implant in a reasonable amount of time.
When implants deflate, the breast tissue begins to contract. I usually recommend replacement within the first 6-8 weeks following rupture.
Rippling in saline implants
The issue of rippling isn’t really about the size even though large implants thin the tissue more and may cause more rippling.
The muscle doesn’t cover the whole implant either so it really depends on how thin you are and if the implants can be overfilled a little bit.
Ruptured salie filled implant
If a saline implant leaks it is not a health risk but the implant should be replaced as soon as possible. As the implant deflates the capsule can begin to get smaller. This would convert a simple implant exchange, that can be done under local anesthesia, to a procedure where the capsule needs to be enlarged to accommodate the new implant.
Am I going too big.
The breast size you choose depends on available chest width and skin envelope to that the implant chosen blends with your breast. Take time to deterine what look you are after as it can be very deceiving to “try on” breast implants and read the correct size.
If your surgeon knows just how full you wish to be and the look you wish for he might be able to use a sizer during your procedure to determine the best implant for you.
Implants with “Fibrocystic” Breasts
There is no such thing as fibrocystic breasts. This is just a verbal shortcut physicians take when talking about lumpy and, frequently, painful breasts. Breasts with these conditions have no more chance of developing breast cancer than breasts without pain or lumps.
There is a difference if you have had a biopsy and the pathology showed one of several very specific changes. However, if the pathologist wrote “fibrocystic changes,” this tells you nothing about your risks. The only problem with lumpy breasts is determining which lump may be cancer.
Therefore, you should be having diagnostic mammograms (3 views minimum) rather than screening ones. According to a very large, multicenter study, there is no difference in breast cancer detection in someone with implants, when compared to someone without implants, if diagnostic mammograms are used.
Your plastic surgeon has, obviously, read this and other pertinent studies. Post op, make sure the mammograms are “diagnostic.”.
Depends on your size and weight
The best thing I can say is look at your doctors pictures and find your size and frame and look at the results. With almost 20 years of cosmetic surgery experience I have found that by listening carefully to my patient describe what size they are looking for and then having them look at my before and after pictures we can pick the right implant for them.
I am surprised some doctors do not have may pictures to show which is a bad sign.
In general, at least 2 weeks off smoking before breast augmentation
Smoking could adversly affect wound healing, post operative recovery in general. An important factor is the degree of dissection needed for the breast lift. For procedures that require minimal dissection (e.g eyelid surgery, breast augmentation, etc.), I ask for 2 weeks at least.
For more involving procedures like tummy tuck, I like to have at least 6 weeks. The data in the literature is not clear about the duration needed for the micro vascular changes to be reversed.
I also modify my surgical technique for smokers so less undermining for face lift, tummy tucks, etc.
But even that is inaccurate. There is no technique outside of the operating room that can give you the exact size of the implant needed to achieve your goals. The best way to get what you want is to give your surgeon the vision you have for yourself and then he/she can work their best to achieve that result regardless of implant size.
Breast implants over the muscle best to correct slight sagging.
Over the muscle best option for weight lifters also. With good surgery, you can get natural result.
Tear Drop vs Round Implants
I tend to prefer round implants. I am still concerned about implant rotation creating a poor result. However I do think it is reasonable if someone has very little breast tissue and wants a natural result.
In this case you need to make a tight pocket to prevent rotation. Most of my patients are often looking for more upper pole fullness and tend to go with round high profile. The tear drop is flatter on top and that is often not my patients goals.
Breast augmentation
Generally, you should not expect the implant to have much effect on the nipple or nipple-aeolar complex, even with a periaeolar incision. While an inverted nipple might look a little better initially, it usually goes back to its originial shape and form when the post-operative swelling resolves.
If this is something you would like addressed it would be best to have it done as a separate procedure and not count on the implant to correct it for you.
