Textured and smooth breast implants

One of the keys to a good patient-physician relationship is good communication. Speak with your surgeon and find out the reason for the change in the plan. In general, textured implants do not offer many benefits over smooth implants.

Some surgeons believe there is a lower capsular contracture rate with textured implants. This is most true when the implant is placed above the muscle. Textured implants are also more useful when an anatomic (shaped) implant is chosen versus a round implant.

The texturing helps prevent the anatomic implant from moving around which is important since there is a specific shape to it versus a round implant. However, texturing can have some downsides such as increased palpability and higher rupture rates. In general in our office in Bloomfield Hills, we use smooth round implants for breast augmentation.

Augment or lift

Although you would like an augment only, the problem is the degree of ptosis already demonstrated by your breasts. Even though you are young, your breasts have stretched out while you were heavier. My advice is either to have the appropriate operation or not at all.

Don’t try to do something halfway which will leave you dissatisfied in the end.

339cc implants and breast size

The best test for you at home is to take some rice and place it in nylon stockings. Grams are about the same volume as cc. Put 339cc (ie grams) in one stocking and 371 cc (ie grams) in the other stocking.
This amount of precision is not possible with plastic surgery, but we, as many others, size you in double bras (tighter on the outside, and insert implants until the look you like appears). Do not force the surgeon to place exactly that volume, for at surgery, conditions change.

He or she will determine the best size and balancing for your frame. We never proceed to surgery without the double bra test repeated at least twice, and have some latitude provided by the patient.

I like saline implants for the type of que400cc or in high profile to about 425cc, a 375cc implant can be filled to 425cc and so on. A C cup is considered in the range you seek but with shoulder width, chest breadth, and vertical distance of the chest, and how far apart the breasts are, these factors can yield a cup size of (32 full C to 36 BC).
How long after stopping breast feeding can you have breast augementation

I agree that waiting at least 3 months from cessation of breast-feeding prior to breast surgery. The concern is that you may start lactating more with the surgery and the massage.

Implants or Lift for Saggy Breasts?

Although you could just have implants, your breast shape will be greatly improved with a breast lift. Congratulations on the weight loss! It is very common to have breast ptosis (sagginess) after weight loss. A breast lift along with implants should give you a very nice result.

Definitely meet with a board certified plastic surgeon to find out more.

A natural feel to breasts with implants

Most breasts with implants feel more natural if they have a significant amount of breast tissue or fat around the implant. If the woman is thin, with thin tissues and not much breast volume, the implant will constitute the majority of the volume of the breast and it is very unlikely that it will feel “normal”.
The most natural feel to breasts with implants is obtained with a correct match between the volume of the implant and the existing volume of the breast tissue and fat, not to exceed a 50/50 proportion.

Fat transfer can be performed very successfully in addition to implants for a more “natural” feel, and it is currently used to treat problems like rippling, palpable implants, and thinning of the breast tissue after biopsy or other reconstructive procedures.

Breast creams

At best, these approaches will cause swelling similar to normal hormonal fluctuations such as your menstrual cycle. Do not waste your money.

800 CC’s?

Sounds like the jury is in… but it comes down to what your want and what you are willing to accept in terms of risk and long term issues. I hope you were able to resolve your concerns with your doctor and that you are able to get what you want.
800 cc’s will be large on you, obvious, and produce consider upper pole fullness. Depending on the dimensions, it could also make you nipples appear like they are pointing outwards.

You have two things to think about. lift vs no lift- this depends primarily on your post operative expectations. Looking at your photos, you do appear to have a nipple level above the level of your inframammary crease.

In my opinion, you probably would be OK without a lift but will still have some pseudo ptosis ( you would not pass a pencil test- but if that is your expectation, then ok) You can always do a lift later.

which surgical placement site is best for you. In general, I think that patients that are borderline droopy do better in a plane above the chest muscle because the breast soft tissue envelope drapes better.
It is not as simple as above or below. There are actually 4 planes for placement of implants. Subglandular, subfacial, partial submuscular (duelplane), or total submuscular. Which plane is used depends on multiple factors, but the most important is how much soft tissue coverage you have.

In patients with adequate soft tissue coverage, I find the subfacial plane will give the best aesthetic result and avoid animation deformity.

Bra Shopping After Augmentation

I recommend to my patients they wait until a full month after surgery. You will be too swollen before that time to get a proper fit when selecting bras.

Silicone does not cause any illnesses.

It is certainly not an emergency, but ruptured implants should be replaced. I would not do aggressive surgery to remove all the leaking silicone, specially in your lymph node. Leaving a little silicone in your tissues is not going to hurt you.
Will My Breast Get Bigger?

You have a very nice result. Your breasts will not get larger. If anything, they will go down a bit in size. You are still swollen and the breasts haven’t settled at this point. I don’t take after pictures until my patients are six month post-op as there will be subtle changes over several months.

I would relax and not to worry implants don’t get bigger.
