Thank you for your question. Looking at your photo, it seems that you have both excess fat and skin. Liposuction alone will only address the fat and you will be left with significant skin excess. I suspect that you would need an abdominoplasty. Please do go and see a board certified surgeon for a full assessment.

You raise two important issues here.
Being paraplegic is not a contraindication to an abdominoplasty. However, the risks versus benefits needs to be considered and you would need a full medical work-up for this.
Regarding the muscle atrophy, surgery will not improve the ‘atrophy’ but may tighted the gapping between the rectus muscles. The atrophy would need assessment to see there is any other pathology and whether this requires repair with a sutures only, mesh or another…

Exercise is of most benefit to your overall health and well-being. Unfortunately, loose skin and gapping of the rectus abdominis muscles (diastasis recti) cannot be completely corrected without surgery. An abdominoplasty (tummy tuck) will help remove the excess skin and tighten the muscles too. I would recommend seeing a board certified surgeon for a full review. Good luck with your journey.

This is an important question. Generally only a few patients are appropriate for a mini-abominoplasty – those with a small amount of excess skin below the umbilicus (belly button). Looking at your photos, it seems that a full abdominoplasty would be of more benefit. It would be useful to assess your tummy muscles (rectus abdominis) and gap between them (diastasis) to see if you would benefit to repair this too. This is a really important question that I often get asked in clinic. I would certainly recommend delaying an abdominoplasty (tummy tuck) until after your family is completed. Being pregnant undoes a lot of the benefits of a tummy tuck. With regards to liposuction, this is more difficult to assess on a photo.
