Both colleagues suggested different approaches which are both reasonable. I would like to inform you about another option. Polyurethane sheet covered silicone implants do not conclude with capsular contraction which are one of the best options in secondary cases like yours. Those implants are also very strongly attached where they are implanted and resistant against gravity. I suggest you to consider this option additionally.

I believe implants settled in different levels under muscle. Sometimes implants may be trapped at lower portion by lowest fibers of muscle and pushed higher this results with asymetrical implant locations. Once implant is trapped by muscle, due to contraction it is common to see implants moving to lateral side. I guess this is your case. There are different solutions to correct this situation. I would like to inform you that scar healing process takes nearly 12 months after any surgical intervention. Staying away from smoking, avoiding direct sunlight exposure to scars and using SPF50+ will also increase your scar quality. I suggest you to be patient and obviously you are using scar recovery gel which is good.

There are various techniques to perform midface lift. Temporal hairline incision, temporal inside hair incision, subciliar incision(insicion below eyelashes of lower eyelid), intraoral incisions are most common incisions. Most of surgeons combine more than 1 incision to get best results. My main approach is using temporal incision inside hairline 3 cm and intraoral 1.5 cm incision. Of course lower eyelid corrections can be made using subciliary incision if necessary.

I believe you are experiencing waterfall deformity. There are 2 ways to solve this issue. First, is releaseing muscle that is holding the implant like a hammock in high position on the chestwall.
