Breast base diameter is important

Without an exam it is difficult to say whether or not a 650ml implant would be appropriate. The breast base diameter is an important consideration when compared to the implant diameter.

The two should be close to offer the most natural result.

Further projection can be achieved with a plus profile implant or even high profile.

Placement of implants in a bra offers some guidance in terms of volume, although when placed in the body, some volume loss is appreciated from the demonstration and this should be considered.

Either way, a 650ml implant is on the large side and desires and outcomes should be carefully considered.

Are my areolas large enough for periareolar incision?

You can have a periareolar incision for breast augmentation. The implant will need to be placed through a Keller funnel to keep the incision line to a minimum.

We find the periareolar scar the best and most predictable in terms of healing, some cannot be seen. With a no touch technique complications are uncommon.

Can I get high profile implants to increase volume and size? Or would I need a lift too?

Breast Implants don’t usually cause stretch marks

This answer seems obvious but it is in fact very rare to have breast augmentation cause breast stretch marks. In those individuals who do not have stretch marks, I am not aware of implants causing stretch marks in well over a thousand patients.

However, In those individuals with pre-existing stretch marks, I have seen two responses: In the vast majority, the stretch marks are pulled and stretched and appear less noticeable. In a very small number of patients (3) I have seen the marks become temporarily reddened over a period of 3 months after which they subside.
Breast Augmentation and Mastopexy

It is always difficult to judge what type of mastopexy is required from the photos, but I can say with certainty that you would benefit from an augmentation along with a mastopexy. A mastopexy alone will bring your nipple and breast up to a better location, but you will have minimal volume and I would guess you would not be happy.

A breast augmentation alone without a mastopexy also is not an option as it will not relocated your nipple/areola to a higher location.

An MRI is the most accurate method to detect breast implant rupture.

Rupture of silicone gel implants is usually silent meaning that you do not perceive that the implant has ruptured. This is because your body developes a tissue capsule around the ruptured implant which makes the implant feel normal.
The only way to detect rupture of silicone gel implants is with an MRI study. Saline implants on the other hand deflate when they rupture so you notice a difference immediately in the size of the breast.

Occaisionally a slow leak occurs with a gradual decrease in size. The quoted rate of implant rupture is 1% per year, which means that 1 % of implants rupture each year. Another way to look at it is that over a ten year period, the period covered by most implant warranties, 10% of implants will rupture.

Second consultation

It is not bad ettiquet to get a second consultation, but it is an excellent idea. Two or three ideas are much better than just one, and then you also can have a better feeling of a connection with the surgeon that is going to do your operation.

No need for a lift

You appear to have deflated breasts that would be lifted with large higher profile breast implants alone. The implants can also correct the slight asymmetry as well.

Looking for large breast implants

I’m not sure if you have had a consultation yet, but please do not focus too much on the actual number of cc per implant. The idea is to find an implant that will achieve the look that you want while still fitting your breast width, which is a limiting factor.

If you choose a very wide breast implant on a narrow body frame, the implant ends up bulging into the armpit region and can become a nuisance. I think that you have the right idea that, if you do want larger breasts, a high or ultra high profile implant might be right to give you extra projection and upper pole roundness.

Pain after Breast Implant Surgery

Pain is such a subjective feeling that it is hard to qualify for all patients. In general, breast augmentation is fairly comfortable and a day only procedure. Most women report a sensation of tightness which stands to reason as an implant is expanding the breast region.

Most women are onto simple analgesia like Panadol by Day 3 postop in my experience and often no analgesia by day 5. Aspects of surgery that generally cause more pain are implants UNDER the muscle and where there is a very large increase in breast size.

In these circumstances, I always use local anaesthetic blocks as I find they offer close to no pain for some hours after surgery, by which time my patients can control their own analgesia to their liking.
In summary, pain is very minimal and easily controlled.

Signs of Capsular Contracture

When a breast implant is placed, the body will naturally form a lining around the implant. This lining is generally referred to as a capsule. Normally, the capsule is thin and pliable and the result is a soft and natural feeling breast.

In some instances, this capsule can become thick. When it becomes thick and more firm, the breast becomes firmer in its feel. Sometimes, the entire capsule can shrink, thus compressing the implant and making the breast feel more firm.

The degree can range anywhere from mild firmness to something that is very hard. In more extreme situations it can actually lead to visible distortion of the breast. Some known causes of capsular contracture include bleeding within the pocket or possibly a postoperative infection.

The actual signs of capsular contracture, in general, are progressive increase in the firmness of the breast and, in more extreme situations, as mentioned, the visible distortion of the breast.

Implants, No Lift

As you know, it’s impossible to say for sure without a physical exam, but based on your photos, I don’t think you’d be happy with just breast implants. They will increase the size of your breasts, but the added weight of the implant could actually worsen the sagging of your breasts.

You’re much better off combining the two into one surgery, rather than finding out after your breast augmentation that you really need a lift, too. A lift will remove the excess, sagging skin, elevate the nipple to its youthful position, and generally tighten things up so that you can enjoy your perky breasts for longer.

Some women also choose to have the diameter of their areolas reduced as part of the procedure. You deserve optimal results from your surgery, so make sure you consult with a board-certified plastic surgeon and be very clear with him or her about what you want.

High profile implants can look very natural.

As with most questions about breast implants, the answer is dependent on your anatomy. You describe yourself as small-framed, with a 30-inch rib cage. Based on this description, and without having seen your photos, I think high profile might be the best option for you.

Small-framed women tend to do best with high profile implants, especially those seeking a substantial boost in volume such as yourself. Moderate or low profile implants would likely overwhelm your chest area and create a less natural appearance.
However, if you are concerned with natural looking results, you may want to consider a more modest implant size, especially for someone of your height and weight.
