Breast Implant Massage: Smooth vs Textured Implants

Breast implant massage after augmentation is used to prevent capsular contracture and to assist with implant positioning.

Capsular contracture is a rare but problematic complication characterized by progressive scar tissue formation around the implant. Capsular contracture is graded on a scale from I – IV.

Capsular contracture is noted to be higher for subglandular versus submuscular augmentation. In subglandular augmentation, smooth implants have a higher incidence of capsular contracture compared to textured implants.

New generation textured implants have a very low incidence of capsular contracture and several implant companies offer warrenties against capsular contracture.

For this reason, I almost exclusively use textured implants in my practice. With textured implants, patients do not need to do implant massage due to the low incidence of capsular contracture and because massaging textured implants can cause seromas to form.

According to the shape of my breast, what profile suits me best?

Based on your anatomy you should get an excellent result. I’m afraid you are going to get a wide variety of answers to your question regarding profile, but my preference in patients with you starting anatomy is usually a round moderate or moderate plus profile implant.

Your size range is probably close, but starting with a good amount of natural volume, you need to be careful not to overdo the implant volume.

Not a good idea to place breast implants through a tummy tuck IMO

IMO, it isn’t wise to do this. Breast implant incisions are short and heal well so the accuracy of placing the implants is better using the standard breast incisions rather than from overly dissecting the abdomen unnecessarily to go through that approach.
Besides, you need to anticipate that you might need revision surgery at some time and you will need a breast incision to do that. Sometimes it isn’t the best idea to try to get too cute with the surgery but to do it in a way where there is the highest chance of the best result.

Breast Implants and Pregnancy

Breast implants do not complicate pregnancy or breast feeding and are safe for everyone based on the FDA silicone gel study. Breast changes occur with pregnancy and the glandular tissue increases in size for milk production to varying degrees depending on your genetics and how much breast tissue was present before surgery.

Most augmentation patients need to supplement (as most women today do), but breast milk has many benefits for the baby and is safe with implants.

Pain is more commonly caused by engorgement of milk or rapid breast growth in response to hormones which may cause stretch marks in some genetically predisposed patients.
The breasts can go down to the same shape before pregnancy, but most women will need a lift procedure to reshape the breast on the implant after having kids.

Incision

Periareolar incisions are generally less visible than inframammary incisions. Perithelial incisions, where the incision is placed next to the nipple and within the areola are even more minimal. I prefer an armpit scar, with the implant placed with an endoscopic approach.

I find this is the best scar out of all the options. The scar can be invisible when healed in most patients.

Doesn’t want a breast lift

It is sometimes hard to tell where the nipple is in relation to the inframammary crease (since we don’t clearly see the crease in the photo), and how much breast is overhanging the crease.
If the nipple is below the crease, patients will usually need a lift for a good result with a good contour.

If the nipple is at or above the crease, they usually will not need a lift.

HOWEVER, if patients have excessive overhang, even with a nipple above the crease, it is still possible that the implant will not fill out the lower portion of the breast which will hang off the implant (this is called “glandular ptosis”).

Some surgeons will talk a patient into choosing a much larger implant than they would desire in order to “fill out” these borderline patients and avoid a breast lift. Ultimately, this can be a mistake and lead to furher and more complex problems, such as stretched and thinned out tissue that cannot be fixed.

I would recommend staying with an implant size that you are comfortable with. If you need a lift, whether a minor periareolar lift or a more major lift, then that is what is needed, and wishing it were otherwise will not change the situation.
Again, you may not need a lift. Perhaps a third consultation with a board certified plastic surgeon would be helpful, mention your concerns, and have them explain where your nipples are in relation to your crease, whether you have excessive overhang of the lower pole of the breast that will present a problem with augmentation alone, whether they think you will need to go to a “large” implant to fill out your skin if you don’t do a lift, etc.

Which size?

Your best bet would be for you to see a BC plastic surgeon so that breast measurements can be performed. With the help of your surgeon, you can decide the size and profile of device that you’ll need to achieve natural looking breast.

Ruptured breast implants

Ruptured saline implants can be removed when the patient is ready. The deflated implants may pose a threat if a folded edge starts to push against the skin and cause irritation.

Sizing Implants

You can see there are gazillion ways doctors size implants. I believe the patient knows best what they want to look like. 700-800cc implants are large but they may be what you need in order to look the way you want to.

The way I approach sizing is with a sizing bra and the baggies and water trick. IT WORKS!!! plain and simple. Base width problems are handled with a high profile implant. Call my office for more info.

There is essentially no difference

You’re only adding a little over a tablespoon of volume! It’s mainly a mind game – go ahead and use the 425cc if your surgeon is ok with it. If you aren’t happy with 405cc, you likely won’t be happy with 425cc! When we go back to put in bigger implants, we are usually adding 100-200cc to the volume of the implant, so 20cc is not a big deal.
Ruptured silicone implants should be removed.

I am uncertain from our questions exactly what you are asking. I will try to separate the components of your questions and hopefully this will be helpful. First, should ruptured silicone implants be removed? Yes.

The body forms a capsule which compartmentalizes the silicone implant, so that, even if ruptured, they are contained within this capsule.

However, if they are ruptured, then they should be removed or replaced. In this situation, it is not an emergency, but can be scheduled electively — there is no “rush” to replace them.

The second part of your question asks about silicone in the lymph nodes. Do you have enlarged lymph nodes? If so, have you had a proper work-up including a physical exam, medical imaging and possibly a lymph node biopsy? If not, these are questions that you have to address with your surgeon and primary physician sooner rather than later.

Implant preference

We have a multitude of excellent implants on the market now from reliable manufacturers. There is no single right answer which is why I spend a considerable amount of time discussing implants with my patients.

I agree that smooth silicone is generally an excellent choice but also like the gummy implants.

What lift will be achieved with implants without lift

There is no replacement for an in person exam but based upon the pictures you have forwarded I would recommend against an implant without a lift.

If an implant is placed above the muscle I feel it would only create larger and lower positioned and heavy breasts.
