Dr. Pozner received his medical degree at Mount Sinai School of Medicine with a Distinction in Research, which means he conducted and published the results of an original research project during medical school. He trained for five years in general surgery, completing an intensive internship and residency at Mount Sinai Medical Center. He completed a plastic surgery residency at SUNY Downstate Medical Center. He went on to complete two fellowships: one in microsurgery, at Montefiore Medical Center, and another in aesthetic and endoscopic plastic surgery at the Plastic and Aesthetic Surgery Center of Maryland. While in Maryland, he was an assistant professor of plastic surgery at the esteemed Johns Hopkins Medical Center. He is currently an adjunct clinical faculty member in the department of plastic surgery at Cleveland Clinic Florida.

Dr. Pozner is board-certified by the American Board of Plastic Surgery. He’s a member of the American Society for Aesthetic Plastic Surgery, the American Society for Laser Medicine and Surgery, and the American Society of Plastic Surgeons. He’s also a fellow of the American College of Surgeons.

Your implants have dropped past your normal fold causing a fold malposition. Correction involves repairing the fold with Galaflex or other support

mastopexy/reduction alone to even them out
or
mastopexy/reduction augmentation (with mesh) to even them out and enhance size
or
mastopexy/reduction to even them out and fat grafting to enhance shape

You need support internally. I cant comment on your previous surgeries without more info but I wont do a lift with implants without support these days - currently using galaflex. Make sure you see someone with mesh experience as placement can be a little tricky

I would let them settle a bit more and if still unhappy by say another 3-4 months then revise with galaflex mesh for support. My need to excise a little skin along the fold if they stretch but certainly can use the same implants

Microneedle RF devices like Sylfirm, Morpheus, Genius etc can cause fat loss if the needles are placed into the fat. With more superficial treatments it should not cause fat loss. The Halo laser (we did some of the original studies for this) should not cause fat loss as it does not treat deep enough to hit the fat.

Based on the photos i would revise the lift and leave the current implants unless you wanted a size change
i might use galaflex for support but need to evaluate you in person

You have a fold malposition - the implant dropped below the fold. I would correct this using Galaflex mesh. You could only do the lower side if you were happy with the size

Usually in a revision all of the old scars are cut out and new scars are made. We do free laser scar treatment on all of our surgical scars.

Was the same procedure done each time when the capsule returned?
We have a very specific capsular contracture protocol with a recurrence rate of under 5%. It entails:

complete capsulectomy
submuscular conversion if currently over the muscle
wash with hypochlorous acid and triple antibiotic betadine solution
new implants
strattice
drain
specimens are sent to microgendx for pcr and next generation sequencing and if bacteria positive we put on antibiotics specific to the organisms for 4-6 weeks

Most likely the issue is the lateral pocket was not corrected or not supported. I would repair with support such as Galaflex mesh

The skin and tissue from the nipple/areola to the inframammary fold has stretched. To fix this I would revise part of the lift (probably wont touch areola) and add Galaflex mesh for support. Your tissues cant support your implant - thats why they are stretched. The mesh acts as support to hold the implant up.

In lighter skinned people dark circles are almost always caused by increased veins in the lower lids. The best treatment for these blue veins is a Nd:YAG laser - wavelength 1064nm. Look for an experienced practice with a Sciton clear-v or Cutera excel-v. There are other 1064 lasers but these are modern devices that are less risky due to their cooling systems. I cant stress enough to go to an experienced practitioner
