Breast asymmetry

Everyone woman has some amount of asymmetry – in nature, nothing is perfectly symmetric. However, with surgery we can usually make improvements. Estimating breast implant size before surgery is just that, an estimation. However, in the operating room, we have sizers and many different size implants to try and achieve the best look and symmetry for you while you are on the table.

That is why it is important for you to have good communication with your surgeon so that you both understand your expectations for the surgery.

Breast Implant Without a Lift?

Your result with the tummy tuck will be great, and I’m excited for you and your future self.

I do think you need a lift. A 750 cc implant will simply give you a low-hanging, heavy breast, and you will still have excess skin. A breast lift, combined with a small implant (likely below the muscle) would give you a very nice shape.

I would discuss the plan with your surgeon again.

Lift or not?

Usually I would recommend a vertical style lift in addition to implants. That will also help with your areola size as well. When the nipple rests below the bra line it’s usually an indicator that a more involved lift is needed to properly reposition your nipple and reshape the breast.

Even though the scars are more involved the shape long term is significantly better it never hurts to get a second opinion as well

Very rarely.

You know that you cannot have silicone implants with the TUBA operation, don’t you? About 5% of patients will get slightly firm implants. In Manhattan, we typically don’t do anything about this, because it does not bother the patient.
Sometimes, the patient may even like the firmness. Very rarely after breast augmentation, the implants can become hard, and this has to be corrected.

Silent Rupture and Cohesive Silicone Gel implants

A Silent rupture means that the implant can be ruptured without you knowing it. Compare this to a saline implant. How do you know when it is ruptured? You get a “flat tire”. The saline leaks out and your body tends to absorb it.
This is due to the memory gel consistency of a Mentor implant which some people compare to a marshmallow. Think about slicing a marshmallow in half, the inside will be sticky but it won’t lose its shape.

When older silicone implants ruptured, they tended to lose their shape. Memory implants will not . Therefore the only way on knowing for sure is to do surgery. The second best way is to perform an MRI which is about 89% accurate.

Current estimates of rupture are about 0.5% (1 in 200) at 3 years. This rate and the silent aspect of implant rupture led the FDA to come up with the recommendation of obtaining an MRI at 3 years after augmentation.

Although 199 will undergo the test “unnecessarily”. I hope this helps .

Leaking silicone implants do not cause remote problems and can be replaced at any time.

Your nasopharyngeal are coincidental with the failure of the saline implant, not the result of it. The saline implant will ultimately lose all of its volume so exercise is not contraindicated. Replacement may done at your convenience.

Ditto

I feel that silicone is more natural looking and feeling. No matter which implants you have you can always have a capsule contracture and other problems that change the look.

But in general – my patients and I prefer silicone.

Lift is essential

You have a tricky shape that is not as straightforward as just placing an implant. Your best option is to combine the implant and the lift. If you are on the fence, then perhaps it would be best to wait until you have had children.

Don’t feel pressured into using a surgeon that you are not comfortable with. If you have any issues postoperatively it will be hard to deal with someone that is not friendly!
Start massaging when your doctor feels the time is appropriate. Depending on the surgery and your doctor, that can range from a couple days to over a week post operatively. Massaging will help shape the breast and reduce capsular contracture formation, the dreaded hardening of breast tissue around the implant.

Circumareolar release with areola reduction and fat grafting.

The treatment for the tubular breast involves releasing the constricting scar tissue and adding volume to the cleavage. I would recommend circumareolar breast scar release with areolar reduction and fat grafting . The use of the implant in the tubular breast is the common practice but leads to later breast deformities.
