Breast implants-profile and positioning
With most of my patients I prefer to go under the muscle. When there is more tissue above the implant it provides more camouflage from the rippling. Most of my patients choose silicone because the rippling is less obvious than with saline.
The difference between high profile and moderate plus will change the projection of the implants. Discuss with your surgeon your desired result and ask to see pictures of other women who have moderate plus profile and high profile implants.
Breast Asymmetry after Breast Augmentation
It is quite common to have some degree of breast asymmetry for several months after surgery. I ask my patients to allow for at least 9 months to one year before making any final judgments about the end results of surgery.
My patients are also cautioned that the breasts will never be identical (“sisters not identical twins”). Continue to follow up with your plastic surgeon and try to maintain realistic expectations- that way you are much more likely to be happy and enjoy the results of your surgery.
Implant assymetry
Sorry to hear about your implant issue. Its difficult to tell, but I’m assuming your surgery was recent. It is not at all unusual to to use asymmetric volume in implants to correct asymmetry.
If it is very early in the postoperative period, the asymmetry may indeed be due to swelling. If the swelling resolves, and you are still asymmetric, adjusting implant volume to achieve better symmetry is only a minor procedure.
Don’t be discouraged. Give it some time.
Natural Feel Of Breast Implants ??
The concept of a naturally feeling breast augmentation result varies depending upon the evaluator. Many breast augmentation results do feel quite natural, particularly using silicone gel implants that are in a submuscular position. Saline implants above the muscle in many patients certainly do not feel natural wih their associated rippling and a higher risk of some degree of capsular contracture.
Not having natural feeling breast implants is an uncommon complaint in my experience and more women seem to be satisfied with their feel than not. While adding fat around a breast implant is possible, it must be done as a secondary procedure and runs the potential risk of implant rupture.
It is usually done in cases of breast reconstruction and not cosmetic breast augmentation.
Breast implants do not commonly cause stretch marks
Anytime your skin is rapidly stretched, beyond its limits, stretch marks may result. However, they do not commonly result after breast augmentation. Most stretch marks that are seen after breast implants are usually stretch marks that were already there to begin with, but since the skin was not stretched out, they were not as easily visible.
Most young women getting breast implants have youthful, elastic skin, that tolerates stretching without developing stretch marks. More mature women getting breast implants may have lost breast volume, and replacing it with an implant doesn’t stretch the skin as much.
The women prone to getting stretch marks are those women with tight, thin skin who are getting “large” implants.
Implants do not cause weight gain!
Implants do not cause weight gain. There are obvious reasons for this and obvious soultions! If you lose weight, the breasts should diminish in size. They may not return to their “original 36C. Diet and excercise should assist you here.

Saline breast implants placed over the muscle
There is NO PERFECT breast implant out there and none on the horizon. All implants are made from a shell of silicone which ripples MORE when filled with salt water (saline) than when filled with silicone gel (but both ripple).
In addition, the edge of the implant is felt as well. There in getting the best looking result, the implant has to be covered by as much tissue as possible.
The more tissue covering the implant, the less its ripples will be visible and the less its edges will be palpable.
By placing an implant ABOVE the muscle you will get slightly more PROJECTION but unless the implant is well covered by your breast tissue it will look “DONE” (the ripples will be visible and the edges will be easily palpable).
SO -if you MUST go ABOVE, I suggest you consider a silicone gel implant because they ripples less. By placing breast implants under the Pectoral is Major muscle, we add another tissue buffer between the skin and the implant for a softer look and more cover of the implant edges and ripples.
If you do not have a lot of breast tissue, seriously consider going UNDER. If you have plenty of cover — you can go either way.
Breast sizing systems are available to keep the guesswork out of implant volume selection.
There are good implant sizing systems that will allow you to try on different volumes to see which works best for you. This keeps much of the guesswork out of it.
Areolar incision
It is not only possible, but actually one of the most commonly used techniques. It is often used with a Keller funnel for silicone implants and is often considered a “favored” area to pace the incision depending on skin type and areolar size and pigmentation.
Be sure to see pictures of results from the BCPS with expertise in breast surgery that you choose.
Breast cancer detection is usually only slightly—if at all—compromised by the fact that you’ve had an augmentation. In my opinion, it would be better to place youroimplant beneath the muscle tissue. However, this most likely will make your sag worse and thus necessitate a lift.
The best thing you can do is see a board certified plastic surgeon who can take a look at you and make a recommendation based on your history and your anatomy.
Factors affecting choice of breast implant size
There seems to be consensus among the Real Self contributors that the size should be made between the surgeon and patient together, and I agree, but there are those who have a different approach. One prominent plastic surgeon insists on using “sizers” during surgery to see what fits best, with no input from the patient, and the other extreme is to let the patient pick whatever they want with the rice bag test or something similar.
I think the patient should have the final say but there are anatomic limitations that need to be respected in order to avoid problems.
Old Leaking Silicone Breast Implants Should Be Replaced
After nearly 40 years of presumed good use, your breast implants can certainly stand to be removed and replaced. Although if you are content with how they feel and look, there certainly is nothing wrong with leaving them alone.
Should you desire to remove them, total capsulectomies can be performed with relatively clean removal of any loose silicone. New breast implants can then be immediately replaced if you desire. This is a straightforward procedure that will most likely surprise you on how much softer and more natural new breast implants can be.
Long term effects of breast implants
It is doubtful that if you had waited, you could just have implants placed to improve your breast shape after breast-feeding multiple children. All women’s breasts sag with time as the skin loses its elasticity.
This process is only sped up with multiple pregnancies and breast feeding. It is difficult to know if your breast implants also contributed to this process. Having a breast lift and/or new implants placed is not a disfiguring procedure and most women are very accepting of the scars due to their new beautiful breast shape.
Don’t regret your decision, it sounds as if you enjoyed your implants for many years
Breast Implant Look and Feel
Saline was a fine implant until silicone was released onto the market again. A standard soft round silicone implant has the best feel under natural breast tissue. The next generation out is the form stable implants (gummi bear).
These are firmer, so do not feel as natural, but are shaped with a gradual slope at the top, so they look the most natural. While many woman like a rounded upper breast and the look is popular now, it should not be confused with natural.
A plastic surgeon can show you the different implants, let you feel samples, and show some results. I have a VECTRA system that can give patients a good estimate of what an implant would look like on a patient’s chest.
