You can't really "pop" a mole, but your history and what has come out suggest more of a cystic or pustular acne lesion. You can definitely develop a small cyst at the site of an acneiform lesion that could behave just like this. Pictures might help here.

In order to get a satisfactory results, you're almost certainly going to need to incorporate oral therapy, either antibiotics or isotretinoin (Accutane), into your treatment regimen. You'll need to meet with a dermatologist to discuss the different options, but I'd suggest you do so fairly quickly, these large, cystic lesions are prone to scar.

Using ziana gel alone is a fairly weak regimen, but even so it is not unusual to need 6 weeks or so to see significant improvement in acne after starting a regimen. Tretinoin is a very effective treatment for acne, but it, and all topical products, are often used in combination to treat significant disease. My average patient sees significant improvement during the late second or early third month of therapy. Only about one in three patients has great improvement 4-6 weeks in, in my experience. Stay the course. Accutane is a terrific therapy.

Accutane has not been reported to cause scarring, but you may just be noticing your acne scars more now that there are no inflammatory or cystic lesions to mask them. Once you have been off of the therapy for a year, you can look into some fractional ablative lasering or dermabrasion to improve the cosmetic appearance of your facial scars.

Unfortunately Isotretinoin in any form is considered unsafe for lactating (breastfeeding) women. The available data demonstrates a high risk of adverse affects to the breastfeeding infant. You may be able to start the treatment if you decide to stop breastfeeding though.

You are very unlikely to see an initial breakout from initiation of spironolactone therapy. Realize that with hormonal acne, you are dealing with a disease that has cyclical fluctuations, so there is certainly a possibility that you could have a cyclical flare before the spironolactone begins to benefit you, but the medication itself is not generally associated with a "worse before it gets better" phenomenon.
