Breast Implant Choices
All three manufacturers provide high quality implants. I personally lean toward the Sientra round smooth cohesive gel implants because I believe that they have excellent feel and durability but would never argue that the differences with other brands are significant.
I do not believe in the anatomic or shaped implants since most evaluations with unaware examiners failed to accurately detect which patients received round vs shaped implants.
Having said that, I’ve seen excellent results in patients who received tear drop shaped as well as round implants.
Consequently my advice is always to first select a board certified ps who gives you a sense of confidence and listens to your goals.
Then let him make his own recommendation as to which implant he believes will give you the best aesthetic result in his hands.
Finally, enjoy your results and.
Breast lift
I would say that you would most likely need a full lift together with implants to get the best results. A simple periareolar lift is not very effective when it comes to moving the nipples to a higher position, and you seem to have II degree breast ptosis.
Best advice would be to see a qualified surgeon for an examination in person.
Breast implants after breast feeding
I usualy have patients wait at least four – six months after breast feeding before undergoing breast augmentation. This allows for the milk to dry up and the breasts to involute a bit.
Why not remove under local anesthesia?
Although a deflated saline implant can b e left in place, eventually the capsule contracts and renders the edges of the implant very palpable and painful. We have seen patients who have left their deflated implants in place for up to a year without any problems other than seom pain from feeling the hard edge against a rib.
Having said that, simple removal can be done under local anesthesia in a procedure that usually takes 30 minutes or less.
It is possible to downsize the size of your breast implants
It is possible to downsize the size of your bust and this is not normally a very involved procedure. It involves a surgeon reopening the old scar removing the old implant and replacing it with a smaller implant.
The recovery is relatively quick following this procedure. Overall, 500 cc high profile implants will produce a quite dramatic appearance to the bust as a hope and you may wish to consider a lower profile implant if you would like a more natural look.

Lots of variables involved
I’d agree that the volume of breast tissue you have makes you a reasonable candidate for going on top of the muscle. However, I do tend to stay below most of the time because of the smoother appearance and reduction in risk of contracture.
Your activity level does not change my opinion about choice of location. If you’ve been to a surgeon you trust, then I’d go with his or her recommendation.
Both over and under breast implant positions have benefits.
As you know, breast implants can be placed under or over the muscle. Both of your surgeons are correct. Placing them over the muscle (under the breast tissue only) will allow the implant to “fill” up the the missing volume in your skin and may avoid the lift.
However, long term, the skin may stretch from the weight of the implant an you may likely need a lift down the road. Placing the implants under the muscle gives an extra layer of support to the implant.
If there is a moderate about of ptosis (drooping of the breasts tissue), then you may need a lift to center the breast tissue over the implant. However, the results will likely look more natural, especially in the upper pole with the implant under the muscle.
So, you could go either way and get nice results.
Tuberous breast and gel implants or fat grafting
Tuberous breasts are a congenital malformation that runs a full spectrum of severity and is occasionally considered reconstructive by insurance companies. Women with tuberous breasts were the original patients allowed to participate in the Mentor FDA silicone gel study because gel is most like breast tissue and is needed to reconstruct the lower pole of the breast.
Usually a lower areolar incision is used with relaxing incisions in the dense constrictive band to release and allow the implant to shape the lower pole. Lately fat grafting has been effective in this type of reconstruction without the need for implants at all.
Signs That Breast Implants Are Encapsulated
After an augmentation, the implant should move freely in the submuscular pocket. If it does not then early capsule formation should be entertained.
The capsular contraction can present as simple pain or gross disfigurement with pain.
The treatment, of course, is to remove the capsule and the implant. Capsule removal is essential as is removal and replacement of implants.
The old implants have a biofilm on the surface which contains bacteria that are not removable.
Don’t feel bad for having great breasts!
You got to enjoy your breast implants for many years before childbearing changed your breast shape. Be thankful! Your breasts would likely have required a lift after 4 kids, even if you didn’t have implants, so you haven’t shortchanged yourself at all.
The time to have the breast lift surgery is when the idea of the scars (which usually really aren’t awful, as you describe them) is better to you than your current breast shape.
Implants above or below the muscle?
In general, placing implants BELOW the muscle is better. There is lower incidence of complications such as infection and capsular contracture. Also, the luscle acts as an extra layer of camouflage, so that the implant is not as visible.
This gives a more natural look, especially in someone skinny like yourself. The post-operative pain is more when the implant is placed below the muscle, but the improved results shold make this temporary pain worth it.
I do place implants above the muscle but only in specific situations and this represent only about 5% of my total breast augmentations.
What Happens to Breast Implants on a Plane Trip?
I may not understand the question. Do you mean within 2 weeks after the operation vs after healed? I feel in my patients immediate flying is a problem with added swelling , blood clots, infection risks whether if the operation was implants or a facelift.
But each surgeon determine his own post op protocol. If you are months from the operation than there are no precautions in flying, but getting to the airport 2 hours before hand to wait in lines.
Will Your Breasts Look Larger After They Settle?
It sounds like you like your results. One of the common things we hear after surgery is exactly the question you are asking, “Should I have gone larger?” It is very early in your recovery and things are certainly not what they will be in a couple of months.
The advice you see in the answers here can be summed up by saying, wait a few months, enjoy your new look for now and then re-evaluate in conjunction with your surgeon.
Implant changes
It would be interesting to see your pre-op photos, but more than likely you have asymmetry. The implants will settle with time and you have to be a bit patient. This process can take several months.
Also in terms of rippling in the lower pole, this is can happen when you do not have a lot of soft tissue coverage. Most implants can be palpated there because if they were placed in a submuscular pocket the muscle would have been detached at that spot.
Definitely review your concerns with your surgeon.
