Dear Compassionate4794, Keep in mind that you need 6 months for all the swelling to subside.  Having said that however, there are some possibilities such as a blood collection (hematoma) or fluid collection (seroma) or the muscle tightening suture could have broken or pulled through the muscle.  An imaging study such as an ultrasound would likely diagnose any of those possibilities if a physical exam does not readily reveal the cause.  Best of luck to you.Robert D. Wilcox, M.D.
Follow-up question: Do these show rhinophyma or enlarging pores with age?
Answer to: Do these show rhinophyma
Mar 12, 2019
Dear Newnose2018, This is not rhinophyma but simply blocked pores.  If you google “technique to unblock nose pores,” you will see many home remedies as well as some products you can buy over-the-counter that will take care of this.  Best of luck to youRobert D. Wilcox, M.D.
How long does it typically take for pleating to resolve?
Answer to: How long does it typically take for pleating to resolve?
Mar 12, 2019
Dear AmandaJ55, The first thing I tell my patients on their first post-operative visit before even taking off the dressing is they will have bleeding on the incision and it will take two to three months to flatten out completely.  In addition, it will take six months for all the swelling to resolve.  The reason for the bleeding is the top incision surface is longer than the lower and so the skin has to be bled like that but does resolve.  Be patient and you will be fine.

Is a lift really necessary with my 475cc saline HP implants?
Answer to: Is a lift really necessary with my 475cc saline HP implants? (Photos)
Mar 12, 2019
Dear 2019Iamreadytodothis, I do not see a need for a breast lift whatsoever your nipples are located above your breast folds so they do not need to be raised higher on your torso.  You do have a small element of pseudo-ptosis (“fake drooping” which means a little bit of sagging on the lower breast which means you would pass the “pencil test”).  You are an ideal candidate for a straight forward breast augmentation.Robert D. Wilcox, M.D.
Swelling worse at tip 8 weeks after Rhinoplasty - is this normal?
Answer to: Swelling worse at tip 8 weeks after Rhinoplasty - is this normal?
Mar 12, 2019
Dear Zeina, It is going to take a full year for all of the swelling to go down on your rhinoplasty and the tip is always the last place.  Be patient and it should improve.  Be very cautious about any steroid injections post rhinoplasty.  If your body overreacts to it, you could get thinning of the skin or fat atrophy under the skin.

What shape is the base of an anatomical (tear drop) breast implant?
Answer to: What shape is the base of an anatomical (tear drop)
Mar 12, 2019
Dear Strawblonde91, For most patients the teardrop implant is of little benefit over a round implant, that is because most women do want to have superior pull fullness with augmentation surgery and this is where the teardrop implant is deficient.  I think the teardrop might be a practical choice for a woman with no breast tissue such as in reconstruction.  The other major problem with the teardrop implant is it can rotate after surgery and then give you an odd-shaped breast that has to be su...
Does a silicone breast implant have to be form stable in order to count as a "gummy bear?"
Answer to: Does Silicone breast implants have to be form stable
Mar 12, 2019
Dear Strawblonde91, You will get different opinions from different surgeons on this question.  I believe all three of them count is cohesive gel or memory gel or gummy bear gel.  This is in contrast to the last generation of implants 10 years ago which when sliced with a razor would leak out like syrup.  The current ones hold their shape when cut.  I hope this has been helpful.  The different molecular cross-linking accounts for the difference in how cohesive a gel is.  The best thing for yo...
Am I healing normally? Is my swelling normal for this stage of my recovery?
Answer to: Am I healing normally
Mar 12, 2019
Dear Jadesmi89, I explain to all my patients it takes a full six months for all the swelling to resolve from your tummy tuck and two months for the incision to really smooth out.  Please be reassured, you are very early in your recovery.

Is it normal to have shortness of breath and low blood pressure after getting a septorhinoplasty?
Answer to: Shortness of breath and low blood pressure
Mar 12, 2019
Dear Kingb28, First you did not mention what your blood pressure was or how fast your breathing each minute, it would not be likely to be related to your surgery but low blood pressure and shortness of breath need to be checked out immediately.  It could be symptomatic or pulmonary embolism or congestive heart failure from a variety of causes, even at your young age.

Slipped down stairs 5 days after breast augmentation. Contacted PS, but would like a second opinio
Answer to: Slipped down the stairs 5 days after breast augmentation
Mar 12, 2019
Dear Acasstll0595, The nerve to the nipple comes from the side of the chest under the fourth rib.  It is on tension immediately after a breast augmentation.  You have probably injured it making it hypersensitive.  If you still have sensation to the nipple, then you can assume the nerve is intact even if you are having burning pain in which case time should correct that.Robert D. Wilcox, M.D.
Will adding a graft to the tip of nose project my nose more and if so will it look bad?
Answer to: will adding a graft to the tip
Mar 12, 2019
Dear Jaqvl, It depends what the plastic surgeon intends to do with the tip graft.  He may actually mean an Anderson strut to keep the tip from retracting in, in which case it would not be visible on the tip.  On the other hand, the surgeon may feel to get more definition of the tip, he is going to need a small graft to get the proper shape.  If the surgeon has imaging modalities where he can morph your image on a computer screen, then you and your surgeon can be sure you are on the same pag...
I'm scheduled to get 600cc implants, but I'm not a candidate for submuscular. Any advice or knowle
Answer to 600cc implants
Mar 12, 2019
Dear Considerate4189, The plan you listed is a potential option but I think you are going to be disappointed with the breast on the right side of your photograph.  It appears to be drooping below the level of your breasts fold, at a minimum you should discuss with your surgeon whether a left crescent lift would be warranted.  Best of luck to you.

Will sternal bra prevent symmastia? Do I already have symmastia and malposition of left implant? (
Answer to: Will Sternal bra prevent symmastia
Mar 12, 2019
Dear Grace707, Symmastia is more common when a patient has a concave chest.  As you mentioned, I do not believe you have true symmastia at this time because the skin is not tenting up between the two breast mounds.  When I operate on a patient with the set of circumstances you have listed, I make it a point to keep the implant under the pec major muscle and to be careful not to detach that muscle from the sternum.  The muscles can then actually encourage the separation of the two imp...
