Breast augmentation – where to start?

If you are considering a breast augmentation, I believe the first place to start is by asking yourself what your goals are. This will likely be the first question your surgeon asks you as well.

Things to consider are: Do I simply want to have larger breasts, or am I unhappy with the shape of my breasts? Are your breasts similar in shape and size or are they quite different?

The next step is understanding the process from a patient’s point of view.

Chances are you know other women who have had augmentations. Ask their advice. Are they happy with the result? What would they have done different? Did they choose saline implants or silicone gel implants? And importantly, would they recommend their surgeon to you? For more formal information on breast augmentation, there are many credible sites on the internet which can help you.

First and foremost, I would recommend the American Society of Plastic Surgeons at www.plasticsurgery.org. At this website you can also find links to Board Certified Plastic Surgeons in your area. Both major implant makers, Mentor and Allergan, also have websites which can educate you on the surgery, the implants, and help you locate a qualified Plastic Surgeon for you.

With your lifestyle, combined with the fact that you have a significant amount of natural breast tissue, I would recommend subglandular placement of your implants. You will avoid an animation deformity, have a faster recovery and not lose any pectoralis function.
The disadvantages would be a higher risk of capsular contracture, the possibility of rippling and wrinkling and implant edge visibility. There are several newer gel implants now available that should decrease those risks.

What size breast implant is right for me?

This is a very common question in my office and on realself.com. Breast implant sizing is a little bit of an art, but a lot of it is truly based on measurements. You need the base width of the breast tissue, medial and lateral pinch measurements, upper pole thickness.

These are all very very important in making the final decision regarding size. At that point, you will have a few options within a size range. If you are asking us the difference specifically between a 175 and a 200cc implant, that is difficult to answer.

It is a subtle difference and only you and your surgeon together and figure out which might be better. Having said that, women who are unhappy after breast augmentation are usually unhappy because they chose an implant size that was too small.
Keep that in mind. In terms of location, someone with thin skin and minimal breast tissue (maybe this describes you given the picture) will benefit from placement under the muscle. However, if you are “sporty” and tend to lift weights, your implants will clearly show when you work out since the pectoralis muscle contraction will push the implants out to the side.

You really need to weigh the pros and cons of this with your surgeon and pick a location. If you don’t mind the motion while lifting weights, then under the muscle will likely look more natural for you.

Breast asymmetry will still remain after implants

Trust your surgeon in the judgment of size differential. You should know that no matter what implants are placed and how much your surgeon tries to correct the difference, there will always remain some difference.

Think of your smaller breast as a size 7 shoe and your larger breast as a size 8 shoe. No matter how much you stuff into the size 7 shoe, there will still be the differences in appearance with the size 8 shoe.
Your skin envelope is very different and adding more volume does not correct that completely. Given that, our Santa Rosa breast implant patients who have asymmetry still are very happy with the improvement!.

Breast feeding after augmentation

I tell every patient there is no reason anatomically after the surgery that they can’t try breast feeding. It does not affect the breast ducts regardless of the incision. So enjoy your implants and in the future, enjoy your babies.

I am 5’6″, very thin and my bra size is a 32A. I’m getting 500c saline implants tomorrow. Will my breasts look too big?

The difficult part about answering this question is that the term “too big” varies due to personal opinion and can be different for every patient. This can depend on your anatomy, your dimensions, your goals, etc.
In general, a rough estimate of cup size is 150-200cc of implant. So if you are starting out with an A cup and adding 500cc, then you will be increasing your size to somewhere in the neighborhood of 2.

5-3 cup sizes, so you are looking at ending up in the C+/D. If this is what you were hoping for, then you will hopefully be happy with your results. In addition, you have to consider implant sizing for your breasts as well.

With a 500cc (assuming HP implant), you are looking at a base width of 13.0cm (for Allergan). If your breast width is at or slightly wider than this, then this implant should fit nicely and you should get a good result.

If your base width is considerably less than the width of your chosen implant, this can lead to problems. In the end, the best advice I can give you is to discuss your concerns with your surgeon.
If your surgery is tomorrow and you are not completely satisfied with your choice of implant, surgeon, etc., then remind yourself that this is cosmetic surgery and not a necessity. You should be totally confident before proceeding.

Breast sagginess

It appears that you have substantial ptosis with the lack of upper pole fullness. You would benefit from breast augmentation, but I feel you also require a lift as well. If minimal scars are your objective, then consider a periareolar mastopexy.

This leaves the scar at the edge of your areola. Without a physical exam, it is impossible to gauge exactly how your breast would turn out without the lift, but I am certain they will still have a saggy appearance and get much worse with time.
Do not let the surgeon convince you that very LARGE implants are what is required to avoid excess skin as most patients regret this decision later. Going submuscular without a lift with your grade of ptosis is a mistake.

You need a breast lift

Your breasts are too low and the majority of your breast tissue is in your lower pole. Fat transfer nor implants will correct that. The tissue must be reshaped and re-positioned higher on your chest wall to achieve natural and aesthetically pleasing results.

Standard breast lifts are too weak to achieve lift longevity and upper pole fullness. The Horndeski Method (TM) is a technique that maximizes lift and longevity with natural results without implants or the dreaded vertical scar.

Implants are not a viable option for you as they will inevitably stretch your tissues again and cause more problems down the line. I suggest you visit the breast lift with implant revision forum on this site for the truth about using implants in conjunction with lifts or using implants to ‘lift’ breasts.
Please do your research. This site is a great start.

Breast implant reduction

The risks are similar to what they were in your first procedure. I recommend you wait for at least 6 months before considering downsizing. This allows for swelling to subside and the true results to be more apparent.

You may find that you like your results at that time.

Saline or silicone implant?

There are many factors in deciding between saline or silicone implant. Both implants have been proven to be safe. The incidence of capsular contracture is 1%/yr for silicone and 1-2%/yr for saline implants. The incidence of rupture is virtually the same at 1%/yr.

However, in the case of a saline implant, when this ruptures, the saline (salt water) leaks out and the breast will quickly deflate. In the case of a silicone implant, particularly the later generations of implant, the patient may not even notice as the shape may remain unchanged.
Imaging is required to make the diagnosis. Silicone implants have decreased incidence of rippling than do saline implants. Silicone implants also tend to have a softer more natural feel than saline implants. A slightly larger incision is required for silicone implants versus saline implants.

Silicone implants are more expensive than their saline counterparts. To echo what many surgeons on this forum are likely telling you, silicone implants have largely become the preferred choice for elective breast augmentation due to their more natural feel and less incidence of rippling.

Nonetheless, there are many factors involved in the decision to discuss with a board certified plastic surgeon during a consultation.

If it is comfortable for you to wear an underwire bra, by all means it is perfectly safe. If you are having some discomfort or reservations about the tenderness of your scar or its placement, that is another story.
