Strattice has become a reliable tool to provide soft tissue coverage for implants. I've had good experiences using Strattice to augment the lower pole soft tissue and provide better implant coverage. I've used autolgous fat grafting for upper pole soft tissue augmentation with good results.

In cases like yours, I typically recommend moving the implant to the submuscular position (if it already is not there) and downsizing the implant. Strattice can be used to re-establish a stable infra-mammary fold in patients that have bottomed out on one side.

Strattice is a porcine derived ADM that is used for a variety of breast augmentation revision scenarios. I've had good experiences with using Strattice to address bottoming-out, excessibe lateralization and symmastia problems. It can be used as a 'firewall' for capsular contracture prevention in patients with a history of CC. I've not used Strattive prempetively to prevent CC at time of primary augmentation.

Based on your photos, if you want larger implants, the weak condition of your infra-mammary fold needs to be addressed, or you will bottom-out even further than you already have. I would consider a Strattice sling to create an internal support bra, in your case.
