Great question. Depending on your skin type on the Fitzpatrick Scale from I - VI you may be a good candidate to treat that spot with Pico Way. A better choice would be to use the Zoom handpiece to spot treat and pick the appropriate and safe wavelength based on your skin type. Warmer skin types may be safe to use the nD:YAG 1064nm wavelength on the Pico Way, while fair skinned patients can use the 532nm wavelength which specific targets pigment.

It is important to be very conservative with skin removal for lower eyelid surgery. The risk is that removing too much skin will lead to a change in shape or positioning of the lower eyelid margin. In most people, removing or rearranging the fat and then performing laser or chemical peel on the skin can give a great result. For those with excess skin, then a skin pinch or conservative skin excision may be considered.

Based on your picture, the markings are not made long enough. The incision has to go as far as the skin fold. Otherwise you are going to end up with a "dog ear" with excess skin.
