Above or below the muscle

Most implants are placed under the muscle these days for many benefits – better shape in the upper part of the breast, less implant visibility specially in thin women, significantly decreased capsular contracture rate, improved mammogram results as compared to above the muscle placement.

However, there are women that would benefit from above muscle placement. There is no one answer for everyone. It depends on your specific physical attributes – breast size, shape, skin tone, mobility of your breast over the muscle and whether or not you need a breast lift or not.

Going with a larger implant to stretch your skin may look good in the short term and you may avoid a lift but in the long run a larger implant will stretch your skin more and you will get even more sagging – “a rock in a sock” appearance.

If you skin tone is already causing you to have sagging, adding more volume will only worsen the situation unless you address the loose, excess skin.

If you need a lift, I would do the lift and go with a smaller implant size which will give you better long term results.

If you still have questions regarding the recommendations you have been given, go back for a second visit now that you have heard both surgeons’ opinions and let them justify their positions and why they are recommending one procedure vs. another.

Or – get a third opinion which should help you decide on the correct procedure – remember to think about the long term outcome as well as the short term.

Implant Bottoming Out

It is difficult to give you definitive advice regarding the position of your implant without examining you or seeing your picture. I would recommend you see your surgeon or a Board Certified Plastic Surgeon who can assess the fold competence and provide you with a structured plan.

If it is “bottoming out”, your fold will need to be reconstructed (much like building an internal bra) to support the tissues and bring the implant back up to where it belongs. Judging fold position by where the scar is currently is not the best way to assess the situation.

Are Breast Implants realistic for those who run long distances daily?

Breast implants are quite durable and many aggressive athletes have them. However for your comfort I would suggest smaller implants 300 cc or below and definitely have the implants placed under the muscle. In addition a firm supportive sports bra should arise be worn when running.
In your situation I alawys use subglandular implants

Your concerns are genuine. You should have subglandular augmentation. There are also multiple other arguments against submuscular implants. Simply put subglandular is more physiological and natural approach for breast augmentation.

Breast Lift v Augmentation

If you don’t like the way your breasts look and you just put in implants you will have a larger version of what you don’t like. Looking at your photos you have moderate to severe ptosis(sagging) and without a lift you will get a suboptimal result.

Lift/Implants

I would suggest that you will get a much more satisfying result with a breast lift/implant combination. a thorough in-person examine with a board certified Plastic Surgeon will help you consider your options.
Breast lift and implant?

I do a lot of dual planes to try to prevent a lift, but I clearly feel you need a lift if you are going to consider implants. You have 3rd degree ptosis with your nipples below your inframammary fold.

Augmentation/lifts can be done together. Discuss with your surgeon.

Cup size change

While it is not an exact science, I say between 150 cc’s or so is about 1cup size for most individuals. You can try a baggy test to see for yourself.

Transaxillary approach for gel implants

You are very observant. The biggest problem with transaxillary placment is the malposition of the implants usually too high. This approach was much easier with saline implants. Now with gels, the incision has to be made larger to accomodate the implant.

Yes, plastic surgery tourism has its risks

This is unfortunately something that many surgeons see. The question many patients should ask themselves is what will they do when and if they have a complication following their surgery.

Time will heal

Implants intially placed alsway seem large but the skin is tight and the muscle spasm which make it seem worse. Keep in touch with your plastic surgeon and let time pass before you make a judgement.

Will pregnancy ruin my breast implants

Your pregnancy in itself will not affect or compromise the integrity of your breast implants. Most women who have had breast implants have been able to breast feed without difficulty. Normal breast changes during and after pregnancymay include stretch marks, nipple areola enlargement , skin discolorations, and breast sag.

These are predominantly weight gain and hormonal induced changes and are not caused by your breast implants. Any breast sag or droop secondary to pregnancy can be corrected with a variety of breast lift procedures.

Implant feel

Implants can feel very natural. Generally, the more breast tissue a patient starts out with, the more undetectable the implant is. If an implant is soft, most of the general population would find it difficult to determine whether a breast has an implant or not.
Saline implants feel less natural than silicone.
