The most important thing to consider is first whether this is the best incision to use. Most have moved back to the crease because of the lowest capsular contracture rates. That said, I would also think yours are too small.

You might be able to get the implant in, but the stress on the implant in doing it through such a small opening can permanently dent and damage the “memory gel.” In person exams are critical though for all of these issues to be addressed.

Saline implants

Many of my patients opt for saline based on cost. You appear to have sufficient breast tissue to produce a natural result with placement behind the muscle. If you have any doubts, discuss it again as silicone or gummy implants can always be ordered in time for your procedure.

Moderate plus is your best bet

Moderate plus will be the more natural looking implant for you. High profile implants fill the upper breast very visibly and it sounds as if that is what you are trying to avoid.

Saline implants have more rippling

Silicone implants ripple less than saline implants. In patients with thin tissues overlying the implant, this is especially important because you have less tissue to camoflouge the ripples. In my opinion, silicone is an aesthetically superior implant compared to saline; the downside is that the operation to remove it if it leaks is slightly more complicated than that for saline removal.

I prefer silicone in almost all instances. Patients who have had both saline and silicone prefer silicone, so I would not consider it unfortunate that you were recommended to use silicone. Depending on your chest circumference, an implant of 100-150cc will give you one cup size.
So 300s to be full C with your dimensions sounds right. Either high profile or moderate plus/midrange implants wil work, depending on the width of your breast.

Avoiding lift scars

If you are trying to avoid lift scars, I would recommend silicone implants on top of the muscle to fill out the loose skin and “lift” the nipple/areola complex about an inch. The size choice is fine, I would not use high profile implants, you need wide based implants to fill out your breast “footprint”.

Capsular Contracture 2 weeks post-op?

I am 100% sure this is not a capsular contracture. The scar tissue capsule has not had time to mature to the point of contracture. It usually takes a minimum of 2-3 months before you could even get a CC.

This is more than likely tightness due to pectoral tone or spasm. Talk to your PS about massages you can do to alleviate this.

Age and breast implants
I have patients in their 50s who present for primary augmentation for a variety of reasons. You are not too old to undergo the procedure so long as you are otherwise healthy. See a board certified plastic surgeon for a consultation to discuss the surgery.

Correcting Uneven Sized Breasts

The key to successful surgery is an agreed upon goal. First, pick the size / volume (not shape) of the breast you would like. – If you are happy with with the size of the Left breast, then the right breast needs to be reduced to that size.

– If you are happy with the size of the Right breast, we need to augment the Left breast to the size of the right breast- if you would like an in between size, we would first need to augment the left breast to that size and then reduce the right breast to the size of the new left breast.
– if you wanted BOTH breasts to be larger, intraoperative sizers would need to be used since different size implants would be required in each breast. There are no “rights” or “wrongs” here. YOU and only you should communicate your wishes and based on them you surgeon should educate you on what could and should be done.

ANESTHESIA FOR BREAST AUGMENTATION

The majority of my breast implants are placed under the muscle. For this reason I generally use general anesthesia. I have never kept a patient overnight.

You are right to be concerned. Implants under the breast will make you sag more and could create a snoopy deformity. You need a breast lift with implants. The implant should be placed under the muscle.

Your leaking saline implant should be replaced ASAP.

Of course a leaking implant is not dangerous. But here is the reason it should be replaced right away. As the implant slowly gets smaller, the implant POCKET shrinks around it. If you replace the implant BEFORE the pocket has shrunk, it is a very simple operation; the pocket is fine and you don’t have to do any internal cutting.

You just put in a new implant. But if you wait, and the pocket shrinks, then you have to recreate the pocket, which is like doing the breast augmentation over again.. This is considerably more surgery.

B/C to D cup

Implants look more natural when the ratio of breast tissue to implant is kept low. In other words, the more breast tissue you ahve and the smaller implant you use, the more natural they usually appear.
Unfortunately most women that want implant do not have a lot of breast tissue and often want them large.

Breast Implant Revision / Anatomic Cohesive Gel Implants/ Saline Implants / Under the Muscle / Revision Breast Surgery

The size of implant best for you is dictated by your chest wall measurements. Once we determine that we can choose the profile based on what you want or need to achieve.

If you are seeking a natural look, then the diameter of the implant should be equal to or, more ideally, smaller than the width of your breast. The breast width is a measurement of how wide your breast is at the base, which should be measured at the level of the nipple.

Choosing an implant that is smaller in diameter than your breast width will avoid the “side breast” fullness that is often associated with a more artificial appearance. Other than that, you should choose the implant based on volume, not on the dimensions of the implant.
You should choose a board certified plastic surgeon that you trust to help guide you in this decision.

Silicone will give you a fullness at the top (upper pole fullness). Silicone implants come pre-filled with a silicone gel and are the softest implant available.

They feel more natural, which makes them a good option for women with less natural breast tissue; but they require a larger incision. It may be more difficult to realize if this type of implant has ruptured, so it is important to monitor them with annual follow-up visits.

Additionally, because this implant contains a more liquid silicone (less cross-linked), if this implant should rupture, it will leak only into the scar capsule formed around the implant but may cause some discomfort or implant distortion.
Anatomic gummy bear implants might be a good choice to give you volume. These highly-sought-after, anatomic implants offer a look that more closely resembles the natural silhouette of a breast, and, therefore, are a very attractive option for individuals seeking a natural-looking, aesthetic primary breast augmentation.

Additionally, these implants are an especially excellent option for patients undergoing restorative or corrective breast surgery because they provide more stability, shape, and reduced incidence of capsular contracture. Compared to other types of silicone gel implants, the silicone in the cohesive gel implant is more cross-linked; therefore, should the implant shell “rupture,” it maintains its shape and silicone does not leak.

During your breast augmentation consultation, you should feel the different types of implants available, and try on various implant sizers in front of a mirror to help you to get an idea of how you will look following the surgery.
You should also bring pictures of the look you would like to achieve, as well as a favorite top to wear when trying on implant sizers. The best way to assess and give true advice would be an in-person exam.

It depends on many variables

When I see patients for breast enhancement, I will ask them what breast cup size they are now and what size they want to be. Based on their height, weight, age, whether or not they have had children, whether or not they have breast-fed, whether or not they have had significant weight change, are all contributing factors as to will be needed to achieve the result they’re looking for.
I encourage patients to bring in also to refer them to specific websites including my own where they can see how patients look. By looking at before and after photos of other patients whose breasts are similar to theirs, and whose weight and height are similar, they can get a better idea of what cup size they are aiming for.
