Statement: Dr. Steven K. Struck is a Stanford-educated, board-certified plastic surgeon who has served the San Francisco Bay Area since 1998. At his practices in Atherton, Los Gatos, and Morgan Hill, California. Dr. Struck specializes in surgical procedures of the breast, body, and face. He’s also considered a pioneer at the forefront of skin laser technology. He embraces the latest in medical technology and innovation, often individualizing treatment by combining lasers with surgical techniques to achieve a youthful, natural-looking result.

Scars fade over a period of 6 to 12 months. Fair skinned people usually see faster fading. The process is expedited by placing tapes on the incisions during the healing process, steroid injections into the scar in some cases, and the use of lasers to lighten the color of the scar as it is healing and remodeling.

Patients who are trim with limited breast tissue are at risk for rippling when using saline implants due to the limited amount of tissue they have to cover the implant. In these cases silicone can give a more natural and softer result with decreased rippling.

This is the most common question I get during a breast lift consultation, and the answer is no. An incision is made around the nipple and areola, but the nipple and areola are left attached to the underlying breast tissue to leave the circulation to the nipple intact as well as the sensation.

In the vast majority of cases I place implants below the muscle, because I believe this gives you a softer result. The muscle action on the implant breaks up scar tissue around the implant that can result in hardness of the breast. In trim patients with limited breast tissue, the muscle also gives additional coverage for the implant, which make the breast feel more natural.

I prefer submuscular biplane implant placement over subglandular placement. The benefits of submuscular placement are more soft tissue coverage to minimize palpability of the implant, lower capsular contracture rates and a better mammogram following submuscular implants. Implant movement with muscle action is usually minimal. I have many patients who compete in fitness competitions and they have no problems with submuscular implants.

You do not have tubular breasts. You do have a wide breast base diameter so I would recommend choosing a larger implant which will match your base diameter. A smaller implant might exaggerate the gap between your breasts. I'm sure you and your surgeon will come up with a good plan. Your case is pretty straight forward.

Revision surgery is always more difficult and it is best to take every possible measure to insure the best possible result. Based on your pictures, I would recommend a lift either vertical mastopexy or a traditional wise pattern. The best lift could be determined at a consultation based on specific measurements, but based on your pictures I feel a very short horizontal wise pattern lift will work best. I would probably add Seri to reinforce the lateral pocket and prevent your implants from migrating into the armpit. I believe a donut lift or benelli will only lift the nipple one cm and will not change the side boob or wide appearance. Width can be annoying for trim people like yourself. A vertical mastopexy would lift you better and it would help with width, but I would think about the increased scarring and the fact that it will take about a year for the scar to fade.
