Breast implants can lift your sagging breasts. If the sagging is minimal, this may be all that is needed. However, usually with significant sagging, a breast lift and breast implants may be necessary to achieve a good result.
A careful physical examination and discussion of your option at the time of a consultation with a board certified plastic surgeon is essential to properly evaluate your concerns and treatments.
650cc implants seems huge to me. Are they too big?
They ARE big and heavy. I fully agree with the comments by Drs. Whitt, Yuan and Stratis and would like to amplify on their comments.
For lack of a better language to communicate volume we are collectively stuck discussing bra cup sizes – a VERY inaccurate measuring tool.
After all, ALL women KNOW that they just fit better in certain manufacturers bras than others. Why? Because EACH manufactuers bra cups are different.
If Engineers were to use such divergent and inconsistent measurements no building would have a second floor! To get THE BEST breast augmentation several factors need to looked at before others.
BREAST COVER FOR THE IMPLANT – without adequate breast coverage the implants will look like two grapefruits under a table cloth. How often have you seen this result? So we start by measuring what you have and pick the implant diameter based on this.
SKIN LAXITY – obviously the laxer the skin, the larger or more projecting implant it allows in. Put another way, two identical twin sisters aged 25; one without children; the other had two. Obviously, the latter breasts would allow larger implants.
CHEST WALL WIDTH – the wider the chest flatness, the more likely a larger (wider) implant is to remain ON the chest rather than sliding to the sides when you lay on your back. So the flat area can sway the choice between a Moderate Profile or a High Profile (narrower base) implant AND LOCATION OF THE NIPPLES – the implants need to be placed dead center under the nipples wherever they may be.
In MY opinion a 34 A (meaning not a lot of breast cover) would look artificial with 650cc implants. Moreover, the HEAVIER THE IMPLANTS THE FASTER THE BREAST SAGGING.
Are Breast Implants realistic for those who run long distances daily?
All of us who do breast surgery have a great number of runners among our patients and they do well with implants but most of the very serious runners are careful in choosing implant sizes so they don’t go too big.
Being under the age of 22, are saline implants my only option?
The FDA has placed some “restrictions” on the use of silicone implants in patients less than 22 years of age. There have been a number of patients in my practice under the age of 22, who have anatomical indications that allow for the use of silicone implants.

Breast Implants for Saggy Breasts
It is important to remember that breast implants are used to increase the volume of the breast and only provide a fairly small lift. No particular implant is best for saggy breasts, however, I do favor silicone implants to achieve a more natural appearance and feel to the breasts.
In your case it appears you may have several options. Placing implants above the muscle may give you a small lift and a very ‘natural’ appearance. However, it is more likely that you would benefit from a combination of procedures (Breast augmentation to give you volume in the upper breast and a Mastopexy to lift the nipples to a more ‘perky’ position).
If you choose a combination of these procedures, I prefer to place silicone implants under the muscle. The Mastopexy can be performed with an incision around the areola only or by adding a vertical incision to increase the amount of lift achieved.
Care credit to finance plastic surgery
Just about everyone has felt the pinch of our current economic situation. Plastic surgeons have been hit worse than a lot of other medical specialties because a large portion of our business is cosmetic. Elective plastic surgery is one of the first things many people do without when times are tough.
For that reason, it has actually become even more difficult than usual for surgeons to accept Care Credit. It doesn’t mean we won’t accept it, but it is generally not the most cost-effective option for surgeons or patients.
As many of my colleagues have already mentioned, most plastic surgeons offer a discount for cash-paying patients. Paying cash will generally get you the lowest possible rate on your procedure.
Silicone Ripples Less
It sounds as though your doctor was “spot on!” Silicone implants do ripple less than saline, but thin women may also have some wrinkling with them as well. Also make sure that your doctor places them behind the muscle in order to minimize the wrinkling.
Yes, but give it time
It generally takes around 6 months for the breast implants to “settle in” and all the sweeling to go down to see the result. If, at that time, you feel you would like to be larger, your surgeon could potentially add more saline to your implants as a fairly minor procedure.
The amount that may be added would depend on the implant fill range and how much saline is currently in them. Some degree (10%) or so of overfilling of the implant beyond its manufacturer -stated fill range is generally well-tolerated.
Keep in mind that overfilling too much could cause the implant to ripple and/or feel too firm and unnatural. Discuss the pros and cons of overfilling vs. replacing them with you surgeon before making your decision.
Breasugmentation/ Breast Lift
I commend you for seeking board certified plastic surgeons and for getting more than one opinion. I believe your best result would be with sub muscular implants and a breast lift, it is hard to tell if a peri areolar lift would be enough (without a physical exam).
Implants without a lift?
It is unlikely that you will get a pleasing result by having a breast augmentation without a breast lift. Because the majority of your breast tissue, your nipples, and your areolae are lower than the folds beneath your breasts, you will need to undergo to a breast lift to lift these structures; simply placing an implant will only make thing s look worse instead of better.
Please have a consultation with an experienced Board Certified Plastic Surgeon so that they can explain your options in person and so that you can achieve the improvements in the size and shape of your breasts that you are hoping for.
Breast lift and implants
A photo can not show exactly what you need. It also depends upon the angle at which the photos were taken. An in person exam is essential.

Breast augmentation does not change your risk for breast cancer. You still need to perform monthly self breast exams and get routine screening mammograms (they will get three xrays of each breast instead of two after augmentation).
You need to know that both your self exam and your mammograms will feel/look a little different than before your surgery. You may need a breast lift for optimal appearance even with an implant.
This is something that you need to discuss with your physician.
No such thing as “silicone poisoning”
Additionally, everyone in a modern society has silicone in their system, as it is ubiquitous in our environment in a number of consumer products. If you have ever had an IV, you have silicone from the lubricant on the needle.
A number of blood tests were promoted purporting to be able to diagnose “silicone poisoning” and they have all been proven worthless. If you have seen only one doctor and he/she is suggesting silicone poisoning as a reasonable explanation for your illness, you really need another opinion.
Breast won’t get any larger
Maximum swelling occurs at 72hours so you are past that point. Your implants will probably drop a little and settle out. You have a great result. If you required different implants to account for an asymmetry, your Plastic Surgeon did a great job.
You should just get better and better!
