From this single picture, it seems that you're a very good candidate for this procedure that would aim to remove deposits from the flanks and inner thighs and transfer this tissue to fill the hips. A clinical examination is necessary to estimate the volume of fat tissue available and needed. In the picture attached to my profile you'll see a patient with very similar frame.

I often advice against having BBL in BMI more than 35 as the risks of surgery are much higher (chest infection, DVT/PE, fat necrosis and delayed healing). Also you don't get the best outcome from surgery due to the remaining internal visceral fat tissue. Hi, From the pictures seems that you have fullness as well as skin laxity in the area. Liposuction will reduce the fulness (podge) but will not tighten the skin. A mini tummy tuck can tighten the skin on the lower abdomen only. If you have skin laxity above the belly button, then that would require a full tummy tuck.

Hi, Seems like you have developed a small hernia at the keyhole surgery incision. This should not prevent you getting a tummy tuck at all. An ultrasound scan test is needed to check it. Hernias can be easily repaired during the tummy tuck surgery but may need a small mesh.

Hi Karald, You should be able to have a tummy tuck and muscle repair in the presence of an old mesh but your surgeon should have good experience dealing with hernias and meshes as he/she may need to remove or replace this mesh during the surgery. I wish you all the best.
