Implant size and breast size varies depending on several factors
This is a very hard question to answer, and that is the reason that we have personal consultation before surgery. Implant size and breast size varies based on your chest width, tissue elasticity, and your desires.
If you are not comfortable with the size that your surgeon is offering you, discuss this with him and also get a second consultation.
Breast Augmentation and Breast Feeding
The ABILITY to breast feed, all other factors being equal, MAY be hampered with Breast Augmentation depending on the number of breast ducts and glandular units interrupted by the operation. This does NOT mean that a Breast Augmentation automatically would prevent you from being able to breast feed – just that it may not happen or not be as efficient.
If you are able to breast feed, you will NOT hurt your baby by doing so. Multiple studies failed to demonstrate any adverse effects of breast feeding by women who had breast augmentation.
650 cc breast implants after gastric bypass
When the breasts deflate the treatment is to wither fill them up (augmentation) or to remove the excess skin (breast lift) or both. It is not a simple process to determine what volume will be needed to fill out the deflated breasts so what I do is have patients try on implanst and see what they want to look like.
If I feel the size implant thaty they use is big enough to fill out the extra skin then we do not do a lift. If it is definitely not enough then they will need an implant at the same time.
If it is borderline I caution the patient that they may need or want a breast lift after 3 months. Given your bdoy build and size my concern is that you will have so much weight in the breasts that you will probably stretch out and eventually need a breast lift.
Can you get away with 225cc implants?
They show asymmetric (size and shape) breasts with some constriction and ptosis (droop). Implants do not “lift” breasts — they simply fill the skin brassiere. So adding 225cc (relatively small for your height) implants is certainly possible, but you will likely have a “rock in the sock,” “Snoopy dog,” or waterfall appearance.
None of these is particularly attractive and nothing like your goal photo, which shows a mastopexy (breast lift), which can create a higher, perkier, nicely-shaped result. That being said, your nipples do not appear to be below the level of your inframammary creases, so with enough implant volume to fill your loose breast skin, you do not “need” a lift.
But if you don’t want to be too big, then shaping incisions (breast lift of one of several types) will need to accompany your smaller implant choices to give you something close to your goal photo.
But only if you choose an ABPS-certified, experienced plastic surgeon who agrees to perform what is going to give you the “best” outcome, not the “get by” outcome that ultimately will cause dissatisfaction and probable re-operation.
Your choice. Implants alone can yield a good outcome, but only if you choose implants “large enough” to satisfactorily “fill” your skin brassiere. Larger will help to accomplish this, but if you don’t want larger, then you have to accept the cost and scars of a skin-tightening breast lift and the implant size you request.
A properly-performed breast lift will require the removal of about one ounce of skin, so you must understand that the difference between 225cc and 300cc is only 2 1/2 ounces (or less than a quarter of a can of soda pop).
Not much of a difference!Based on the information provided, I would likely recommend a breast lift plus small implants.
Breast lift
You are a candidate for a breast lift. You can always have an augmentation first. However, you will more than likely not be happy with the result and want a lift as well.
I don’t think you are a candidate for breast augmentation without lift
Unfortunately, your breasts have too much ptosis, or sagging, to allow a very nice result with just breast implants. Even really large implants – 700 or 800 cc – would only make the breasts larger and heavier, and the nipples and breast tissue would still be sagging.
I’m sure you can find a surgeon who will do just this, and as long as all you want is to have your breasts bigger, that might be a reasonable recommendation.
However, most ladies also want a nice shape and position to their breasts too, and in order to achieve that, you will require some form of lift as well as augmentation.
Consult with a number of board certified plastic surgeons who have experience in all forms of breast surgery so that you will be able to learn about all of your options.
In addition to being certified by the American Board of Plastic Surgery, the organization which certifies plastic surgeons, your surgeon should also participate in the Maintenance of Certification program administered by that board. This program requires that surgeons who participate continue to update their credentials and professional standing regularly, as opposed to practicing on a single lifetime certificate like those plastic surgeons who elect not to participate.
To find further information about this program and any surgeons you might consider for your surgery, I suggest visiting these sites: abplsurg.org and abms.org.
Breast lift
Your breast tissue touching the chest wall and nipple is below the infra-mammary fold. If you get implants alone, your breast tissue will slide off the implant and will look worse. The other problem is that you have a lot of excess skin and to fill that will require very large implant.
High Profile Implants?
Based on your ideal pictures, I believe that the HP implants will give you a better chance at achieving your goals. All of your ideal pictures have a fair amount of upper breast pole fullness and the HP implants do the best job at maintaining that look.
I would discuss this in person with your board certified plastic surgeon and see what they recommend.
Choosing implant size is difficult
Without photographs, measurements and an in depth consultation, we can only guess what the end result will look like. Please see a Board Certified Plastic Surgeon who performs breast enhancement procedures frequently to assure yourself of the best possible result.
Breast Augmentation and Lift
I would advise submuscular implants as this will provide for greater longevity of result and less tissue thinning. This also provides for a more natural superior pole. I think I would recommend at least a circumareolar lift as well.

Petite Frame, Best Implant
Women with smaller body frames and especially those without much breast tissue pose special challenges to ensure a natural breast augmentation result that has a lower risk of problems. This really requires an in-person consultation to assess your tissues and learn about what size and shape of breast you desire.
Implants come in a wide variety of sizes and shapes and an experienced, along with a careful in-person sizing process is the best way to ensure the implant “fits” you properly and meets your expecations.
Without an examination or at least, several photographs it is impossible to give you but a generalized set of opinions which may or may not be applicable to you.
First – if you like your current bra cup (despite the sagging) then, a Breast Lift ALONE (especially a short scar Hall-Findlay type procedure) would give you the perky breasts you are looking for.
In the unlikely event that you realize a few months from now that you want MORE volume, you could have a Breast Augmentation at a later date and guess more accurately to the size of the implant.
Second – if you WANT MORE VOLUME now then you need a Lift / Augmentation procedure. Personally, like several of the other surgeons on this panel I prefer that patients understand the CONCEPTS underlying these procedures than their obsessing needlessly over the cc’s in each implant.
After all, it is not the cc’s that would determine the breast appearance but the combination of several other factors as well.
Saggy breasts fixed without lift
I concur that implants alone are not going to elevate you nipple areola complex to a desired level and the implant its to big for your dimension and its not going to resolve your problem.
Safe to fly with breast implants
Implants are not, as far as I know, affected by altitude or high pressure. I understand your concerns in regards to limitations and rules to follow after having breast surgery. There are no limitations when having breast implants.
I have many patients flying in for surgery for breast augmentation and they are able to fly home within a few days. During the recovery period, it is important to avoid any heavy-lifting and strenuous activity for the first few weeks after surgery to avoid implant malposition and/or discomfort.
