Hello, thank you for question. It seems in your pictures that you have breast ptosis (breast falling); so if an implant is placed without performing a lift, the implant will result in ptosis worsening, because the implant is extra weight added to your breast tissues weight, so; if you already have ptosis, expect worse ptosis and results after just placing an implant.

Most of the time in suggesting an implant size a surgeon take into account the breast base measurement; as it is the most important and limiting measure in implant choice; it is not advisable to choose and implant with a base larger Than the breast base; as the corners of the implant could become visible. Also, tissue elasticity and cushion are take into account; as they are limiting in containing certain implant sizes.

An internal bra will push the implant up and medially, when we a re performing a breast augmentation this is desirable; but in breast lifts, if you have very saggy skin; the implant could stay high and the breast tissue slide down, giving you a waterfall deformity. In order to decrease the risk of this, most of the tissue in the lower pole should be resected; this also will decrease the chance of bottoming out.
