Although your breasts are significantly ptotic (sagging), I would not perform a free nipple graft. For patients such as yourself, I use a specific breast reduction technique that preserves blood supply to the nipple/areola, and this has served me very well. However, there are some instances in which a free nipple graft is the ‘safer’ option e.g. previous breast surgery, previous radiotherapy.

Hello
Looking at your photos, it is clear you have necrosis of the skin at the vertical scar. As the necrotic tissue is wet and the surrounding skin appears inflammed I would recommend the necrotic tissue is removed. The subsequent raw wound will need to be washed and dressed regularly to encourage wound healing. Scar revision can be performed at a later date once the tissue has completely settled and the scar matured.
Please see your surgeon as a matter of urgency.

If you have a change in your breast shape in association with pain, I would recommend you have this investigated with imaging, either an ultrasound scan or MRI. The imaging will help determine the state of your implants and/or reveal any other issues in the breast. Consult with a GMC-recognised Plastic Surgeon and he/she can assess you.

Four weeks post op is too early to make a definitive judgement about your breast enlargement. Do keep your appointment with your surgeon and discuss your concerns with him/her. You will not see the final result from breast enlargement until at least 3-4 months have passed.

You will not see a significant difference by over-tightening the rectus muscle separation and is something I would not recommend.

Looking at your photos, it appears you have mild lower eyelid (medial) fat herniation and the junction between the lower eyelid and cheek is quite visible. Camouflaging the fat herniation and tear-trough with fillers or fat grafting can be done but it does not address the underlying issue.

Hi tspinks06, breast enlargement with an implant will certainly help you achieve the larger bust size you desire. Trying to achieve a specific bra size with a particular implant is risky, as bra sizes differ greatly between manufacturers. A 200cc implant may achieve a C-cup with one bra but could result in a D-cup with another bra!
