The effects of radiation are chronic and may go on for many years. This has to do with fibrosis of the tissue, leading to decreased blood supply which may atrophy or shrink over time. Weight changes can also affect the symmetry between the breasts, as the non radiated breast usually changes more with weight gain.

The reason for a waiting period after the last expansion before replacement of tissue expanders with implants is that the tissue needs to have time to stretch and accommodate to the increased size. I usually prefer to wait around 6 weeks for this process to occur. Of course, each case is unique so it is important to speak to your board certified plastic surgeon.

This is an excellent question asked by many of my patients. In fact, this is something that i wanted to improve for my patients and now routinely connect the nerves when performing DIEP flap reconstruction. I have published by research to show that this does indeed help to improve the sensation. I believe this is possible, and encourage more surgeons to provide this option to their patients.

The pain you are experiencing is probably due to the pectoralis muscle that is being stretched by the tissue expander. The newer technique of placement of the tissue expander and coverage with acellular dermal matrix may be possible to treat this issue. The tissue expanders potentially could be placed above the muscle and expanded to accommodate your desired breast size.

From the symptoms that you describe, the cause of your tigtness is most likely the pectoralis muscle that is being stretched by the underlying implant, in combination with capsular contracture. Simply exchanging the implant will not resolve this problem. Yes, placement of implants under a DIEP flap reconstruction is a common method of “augmenting” the volume of your breast reconstruction. In your case, the DIEP flap lost volume with weight loss and fat injections alone may not be enough to give you the desired fullness.
