Breast Shrinkage 10 Years After Radiation for Breast Cancer?
Shrinkage after Radiation
Dec 30, 2017
Dec 30, 2017
The effects of radiation are chronic and may go on for many years. This has to do with fibrosis of the tissue, leading to decreased blood supply which may atrophy or shrink over time. Weight changes can also affect the symmetry between the breasts, as the non radiated breast usually changes more with weight gain.
What is a Typical Timeframe To Keep in Tissue Expanders After Final Fill?
Tissue Expanders
Dec 30, 2017
Dec 30, 2017
The reason for a waiting period after the last expansion before replacement of tissue expanders with implants is that the tissue needs to have time to stretch and accommodate to the increased size. I usually prefer to wait around 6 weeks for this process to occur. Of course, each case is unique so it is important to speak to your board certified plastic surgeon.
Fat Transfer for Correction After Mastectomies. Recovery Time?
Fat Grafting and Breast Reconstruction
Dec 30, 2017
Dec 30, 2017
Fat grafting has become increasingly popular to improve the results in breast reconstruction. This procedure has a relatively fast recovery, and the main area that is affected is the donor site where the fat is taken from using liposuction. There could be bruising and swelling for up to two to three weeks. Compression garments are important to help minimize swelling and help skin elasticity. If you are going to a beach vacation those are some factors to consider.
How Much Fluid Should Be Draining in a Day in Order for my Jp Drains to Be Taken Out?
Breast Reconstruction and Drains
Dec 30, 2017
Dec 30, 2017
I usually like to see less than 25 cc of fluid per drain per 24 hrs. The thought behind this amount is that the body is going to be able to resorb less than that amount, thus preventing fluid accumulation which can lead to developing a seroma.
How bad is numbness after DIEP reconstruction?
Sensation after DIEP flap reconstruction
Aug 20, 2017
Aug 20, 2017
This is an excellent question asked by many of my patients. In fact, this is something that i wanted to improve for my patients and now routinely connect the nerves when performing DIEP flap reconstruction. I have published by research to show that this does indeed help to improve the sensation. I believe this is possible, and encourage more surgeons to provide this option to their patients.
After Bilateral Mastectomy, my Expanders Are Excruciating
Pain with Tissue Expanders
Aug 20, 2017
Aug 20, 2017
The pain you are experiencing is probably due to the pectoralis muscle that is being stretched by the tissue expander. The newer technique of placement of the tissue expander and coverage with acellular dermal matrix may be possible to treat this issue. The tissue expanders potentially could be placed above the muscle and expanded to accommodate your desired breast size.
Breast Implants After Lumpectomy and Radiation?
Implants after lumpectomy with Radiation
Aug 20, 2017
Aug 20, 2017
Radiation therapy causes fibrosis of tissue, hence the breast usually shrinks and may become slightly stiffer after radiation. So when deciding on augmentation with an implant after radiation there are several aspects to consider.
1. Implant related complications increase after radiation, specifically problems with capsular contracture
2. The asymmetry between the two breasts may be magnified with an implant, even if the implants are carefully chosen for size and shape,…

What is Lipofilling and How is It Used for Breast Reconstruction?
Lipofilling in Breast Reconstruction
Aug 20, 2017
Aug 20, 2017
Fat grafting techniques have grown in popularity over the past decade. This can range from being used for total volume breast reconstruction by a few surgeons, to being used to correct minor irregularities and to increase the thickness of tissue over implants. It is a very helpful adjunct to improve results both in autologous and implant based breast reconstruction.
How is Alloderm Used During Breast Reconstruction?
How is Alloderm used in Breast Reconstruction
Aug 20, 2017
Aug 20, 2017
Thank you for your excellent question. Alloderm and other acellular collagen products have revolutionized our results in implant based breast reconstruction. Historically, when implants were first used in breast reconstruction, they were put under the skin in the breast pocket. However, this resulted in complications mainly related to capsular contracture, or thickening of the tissue that the body naturally forms to cover the implant. To prevent this problem, surgeons then started to place…
Are There Nipple Implants for Breast Reconstruction?
Improve Implant Shape for Nipple in Breast Reconstruction
Aug 20, 2017
Aug 20, 2017
You ask an excellent question. Unfortunately we are limited in the projection and shape of implants. We frequently wish we had a choice of implants with more projection to achieve a more conical look as the foundation for nipple reconstruction. Implants were initially designed for breast augmentation, hence they lack that tip projection that would improve the shape after reconstruction. Even the newer shaped implants do not fully match the projection of a natural breast in many cases…
Breast Reconstruction After Radiation?
Radiation after Breast reconstruction with implants
Aug 20, 2017
Aug 20, 2017
As previously explained by my colleagues, radiation has deleterious effects on the breast skin, and causes fibrosis which usually results in tightening of the skin. There are different approaches to the steps taken in breast reconstruction with implants in the setting of radiation. I prefer to leave the tissue expanders in place during the radiation process and exchange them for implants 6-8 months after the end of radiation. By this point much of the effects of radiation have taken…
Tightness, Pulling and Pain After Breast Reconstruction- What Can I Do?
Tightness in chest after reconstruction with implants
Aug 20, 2017
Aug 20, 2017
From the symptoms that you describe, the cause of your tigtness is most likely the pectoralis muscle that is being stretched by the underlying implant, in combination with capsular contracture. Simply exchanging the implant will not resolve this problem. It is now possible to change the location of the implant to be over the muscle and line the breast pocket with collagen to prevent capsular contracture. It is best for you to be evaluated by a board certified plastic reconstructive surgeon…
Risks of Breast Reconstruction with Implants After Radiation Therapy?
Risks of Reconstruction with implant after Radiation
Aug 20, 2017
Aug 20, 2017
Thank you for your question, as this is a frequent point of discussion when patients are deciding on reconstruction options after radiation. The procedure options mainly depend on the amount of breast skin and tissue preserved after the mastectomy, hence the answers are different for specific situations.

Recommendations for the Best Reconstructive Surgeons Who Do DIEP
Likelihood of DIEP flap reconstruction
May 7, 2017
May 7, 2017
Experienced DIEP flap surgeons usually perform a preoperative CT scan or MRI to map out the blood vessels that will nourish the flap tissue. This facilitates preopertive planning to minimize intraoperative surprises. The DIEP procedure can be performed in almost all cases where there is adequate abdominal excess, with rare exceptions where patients may have had previous abdominal surgery that caused significant scarring. Therefore it is important to seek out a surgeon who is…

Implants for DIEP flap augmentation
May 7, 2017
May 7, 2017
Yes, placement of implants under a DIEP flap reconstruction is a common method of “augmenting” the volume of your breast reconstruction. In your case, the DIEP flap lost volume with weight loss and fat injections alone may not be enough to give you the desired fullness. Therefore, implant placement can increase the volume and projection of the reconstructed breast to achieve a more pleasing shape.
