You would benefit from a standard open septorhinoplasty to correct the deviation, to reduce and narrow the bridge and to refine the nose tip. The surgery takes about 2 hours, you would need a splint on the nose after and would be in hospital overnight.

The photos show that you still have quite a lot of asymmetry in the nose and a reasonable degree of deviation with loss of support of the nostril base on the left. Rhinoplasties in patients who have had a cleft lip and palate are undoubtedly the most challenging and difficult cases. You should in my opinion definitely look for a surgeon who specialises in cleft treatments and ideally get a combined opinion from a plastic surgeon (they usually deal with the cosmetic component)…

You should wait about 10-14 days until reusing the Retin-A unless there are any problems with the wounds (crusting or minor infections) as the redness and scaling that sometimes develops may be a potential source of infection. From a long term point of view Retin-A provides an excellent means of maintaining the benefits of the facelift.

When you are thin in the face you really want to preserve or even increase the volume as youthfulness is associated with facial fullness. Removing SMAS will remove volume (albeit only slightly) and it is better to use another means of tightening and repositioning the SMAS such as imbrication, plication or purse-string sutures. Sometimes it is appropriate to add some fat through the technique of lipo-filling.

Really you need to enclose the pre-operative photos. Some asymmetry is almost inevitable and whilst I can see some differences they do not seem very obvious. You can always do minor fiddles on nose tips but getting absolute symmetry is difficult and adjustments can themselves lead to other problems. Without actually examining you and just going from the photographs I would be wary of trying to chase perfection.

It sounds as though you had a classic facelift from the site of the incisions but without further details it is impossible to be sure what exactly was done. Some recurrence of jowls and neck folds is common but it does sound as though this is happening very quickly. Again though without knowing how bad the problem was before it is impossible to advise whether more should have been expected. I do suggest however, waiting for at least 3 if not 6 months before doing anything more as recurrence…
