Ideal had a recall with some lots of their implants. There was a manufacturing issue that allows the fill valve to open, causing deflations. The implant is currently off the market but their warranty should stand as far as covering some costs with replacement. If you want to stick with saline, I would consider another manufacturer.

It appears by the photos that your breast tissue is sagging off the implant position. This may occur with aging, weight loss or simply time. You may need a lift to reposition the breast tissue so it is centered over the implant. A in person consultation will address your issues.

To achieve a tighter and flatter abdomen, a tummy tuck would be ideal. A tummy tuck with lips will eliminate the loose overhanging skin and repair your core muscles. Typically an overnight stay and most patients return to work by two weeks out from surgery. Lipo alone will not eliminate the loose skin and may make it worse. You are correct in getting to your ideal weight prior to the procedure. Good luck!

You do appear to have some minor malpositioning of your implants. If the implants are sitting below your breast fold, a double bubble type appearance may occur and it can cause pain. Capsular contracture usually presents with pain, firmness and typically an upward shift and narrowing of the implant appearance in the pocket. You should expect two different types of results. Lipo will only reduce the thickness of fatty tissue between your skin and abdominal muscles. You may have some loose skin after. Typically, it is safe to do lipo of the abdomen after a tummy tuck. Many patients seek to reduce the thickness of the fatty tissue thats left over after the extra skin is excised from the tummy tuck. It is not currently recommend to do a total capsulectomy and exchange in patients with no symptoms. A true total capsulectomy is not without risk and and should not be taken lightly. To remove a sub muscular capsule in total, can expose one to the risk of bleeding complications,feelablity of the replaced implant and dropping a lung. A in person discussion with your surgeon is needed to discuss the risks and benefits and options so you feel comfortable with your decision

It is hard to tell without an in person exam, however, it does appear your nipples sit above the fold underneath your breast. An implant alone will volumize the breast and fill out the skin but does not truly elevate the nipple position. Depending on your expectations, you may be happy with an implant alone. Seek out an honest opinion from a board certified plastic surgeon.

It would be important for you to know your cause and risk for blood clots. Typically, a thorough evaluation by a hematologist could evaluate your overall risk and discuss medical options to reduce your risk. Most surgeons now use some type of anticoagulant to reduce the risk of blood clots. I would assume after an evaluation that you would be a candidate. Good luck
