Thank you for your question. This consult would need to be done in person with a Board Certified Plastic Surgeon. It would be very important to make sure that your expectations could be achieved with surgery. There is no real way to make your breasts perky without an augmentation. If your desire is to have a more perky and full breast, then a large implant is not the answer.    Your best long term result would include a revision of your lift and a moderate fullness implant.  The shape and size of the implant would depend upon your breast diameter and height.  Trying on sizers would be very helpful to give the visual idea of the final result.  Meet with your plastic surgeon again or get another opinion before proceeding with surgery.

Based upon your photos and history, I feel you would have the best result with a full tummy tuck.  You may end up with a small vertical scar to get the lower scar as low as possible.  One way to avoid this would be a “floating belly button” procedure.  It is an excellent procedure, but only used about 5% of the time, and you need to be the right candidate.  Make sure you seek out a Board Certified Plastic Surgeon for a consultation or 2.  Good luck with your process.

You are very early in the post operative time to consider a revision.  With a vertical mastopexy, such as your surgery, it can change quite a bit in the first 3-4 months.  It will continue to settle and may end up very symmetric.  However, with the different nipple position and size, you may need some revision in the future.  Continue to follow up with your Plastic Surgeon and let them know about your concerns.
