Aloha, it does appear that a lift would be helpful to you. A Vertical/Lollipop or Anchor lift would lift more and last longer. Although you have good size, if you want more upper fullness, you may also need/want a small implant under the muscle, although fat grafting is an option. I recommend that you see an experienced board certified plastic surgeonto review your options after an office consultation and thorough exam. Virtual/3D imaging is also an option we offer.

Based on your history, you might would get the best results with Liposuction and/or a Tummy Tuck; which could be able to be done at the same time. It's not clear from the photos that much needs to be done to the upper bony portion. Your septum probably does not have much effect on your outside appearance.
