I am of the strong opinion that BRAVA for breast reconstruction is not for every situation not every patient. In the right patient, it is a wonderful option with excellent, healthy fat after grafting. Unfortunately the FDA has essentially placed a hold on any further enrollment in the IRB so at this time I am only treating patients already enrolled in the IRB.

Trained Plastic Surgeons go through a rigorous process for board certification. While navigating this process, the surgeon cannot say they are certified. If the surgeon is going through this process and you feel confident in their competency, then application of surgical technique to address your problem should be possible. Now if they are not confident, not listening to your goals, not been successful in passing their board exams, or not plastic surgery trained then I would be concerned….

Both of these implants are improvements over the round implants, in my opinion. When you stand the three upright (round, Sientra, Allergan 410) there is progressively less deformity and folding. The difference between the Sientra and the Allergn 410 is not as dramatic as their difference with the round. In petite patients, particularly, the anatomic can provide a better shape on the upper breast. I personally feel the folding difference between the Sientra and 410 is not as significant…

There will be diminished to no sensation at the mastectomy site. You will not have any feedback as to the temperature of the hot water bottle. I would not recommend it as you are at risk for being burned as well as possibly increasing bleeding/bruising. If you do place something warm there, check it frequently with a part of your body that is sensitive (such as the cheek) rather than skin that is thicker or callous (such as the palm). Be careful.

Removal of dead tissue, especially in the presence of an implant or expander, is important and unfortunately does require surgical removal. If there is partial thickness where the top layer peels off but the deep skin is pink (vascular) this may heal. Turning black, as you have described it, would likely be the full thickness and would require removal. Unless it would interfere with adjuvant (post-surgery) cancer treatment, you may want to consider your flap surgery sooner…

There will always be an increased risk for complications with implants in the setting of radiation (even 10 years later). The difficulties will be with symmetry, capsular contracture, wound healing, and infection. Each result will be different depending upon the severity of radiation injury, the amount of tissue left behind (mastectomy skin thickness), and (in my opinion) the use of a dermal matrix. The short answer is that there is a good chance they will not. There is much reconstruction of the management of Medicaid at this time, which will vary from state to state.
