If you want your breasts lifted, the best option is a vertical augmentation/mastopexy. Small implants (not "anatomic" but round and smooth) are used to restore upper pole volume. The vertical mastopexy (lift) eliminates the drooping skin on the underside of the breast. It is a "minus-plus" operation that produces the illusion of a breast lift. Some plastic surgeons believe that a "dual plane" breast augmentation can take up the skin slack better than a regular "submuscular" breast augmentation. This claim is dubious at best. Yes, the deep plane lift can be focused on the lower face (jowl) and neck. It is a better approach than the skin-only lift. Yes, you are young but you have an unusual problem after your jaw surgery (once I did this procedure on a 15-year-old whose skin had been stretched by steroid treatment). The answer is a full tummy tuck with liposuction. Any time a woman grabs the loose "flub" by her hands, the treatment is clear. Only a tummy tuck will sufficiently tighten the skin, eliminate the roll, and also allow tightening of the underlying muscle. The inverted-T anchor scar should be obsolete because of the very long horizontal scar and other design problems, such as an overelevated nipple. The Benelli is unsatisfactory because it provides no lift and just distorts the areola, sometimes making the breast look like a tomato (this method has fallen into disfavor). It is not possible to know based on the photo whether the rectus abdominus muscles are spread apart. A surgeon may do a proper muscle repair and the abdomen may be protuberant just from intra-abdominal fat that does not go away without weight loss. In my office, we assess the abdomen with ultrasound to determine the position of the muscles. If they are spread apart, you may benefit from another operation to tighten them together ("repair of diastasis").

Can I still get seroma drained or do I have to have surgery?

If a seroma has not resolved within a month or so, it probably won't on its own. Need aspiration in office.

You have had that seroma for almost a year, so it is evidently not going to resolve on its own. In my office, we check any possible seromas with ultrasound, then aspirate them by inserting a needle. You will likely require repeated aspirations in the office. Sometimes it is necessary to inject tetracycline to inflame the lining of the seroma and get it to scar together and stop collecting fluid. Your breasts are already quite large. You look like you are a C cup or even a D cup. 500 cc implants are likely too large for you. That size would make you a DD or DDD. Probably a 300 cc size would be more appropriate. But it also depends on how much breast tissue the surgeon removes when performing the lift. Incidentally, are you sure you need a lift? If you have a lift be sure it is a vertical method, which produces a much better lift effect, nipple position, and more limited scars than...

Actually, I think your result looks good. Not perfectly symmetrical of course but very close. As you know, perfect symmetry is usually not possible to achieve. Plastic surgeons often tell their patients, "The breasts are sisters, not identical twins." I cannot see your abdomen clearly in the photos, so I cannot really comment on whether you would be a good candidate for a tummy tuck or liposuction.

You don't really have classical tuberous breasts. I would recommend a standard breast augmentation using an inframammary approach. Have to be careful not to dissect too much below the crease because the implants will settle over time. Some surgeons intentionally lower the fold but this can lead to a double bubble.

Of course there are plenty of women in Mexico who have facelifts, so it is fine to go to Mexico 3 months, 2 months, or even 1 month after a facelift. Right, be super cautious about UV protection for your incisions and any bruises (unlikely after 1 month). Laser skin resurfacing is a different matter. If you choose to have laser resurfacing you need to be even more cautious about sun protection, depending on your skin type but even after laser resurfacing you can make the trip at 3 months.

Of course you might consider Juvederm in the office, but it lasts about a year. Another option is facial fat injection. Your own fat is the best filler. Usually it is not all absorbed, and we can obtain a larger volume for injection. I do it in the operating room under IV anesthesia. So it is very popular in my practice. You might consider simultaneous laser skin resurfacing. Microneedling and heat treatments are limited in their effectiveness.
