An active sinus infection is a relative contraindication for breast augmentation. You have many options. You need to consult with a board certified plastic surgeon. One optional first step in such a case is the pre-operative deflation of your saline implants. That would allow you to see what your natural breast volume is like and allow the skin to contract a bit. I have a large experience with breast fat grafting. It is critical to understand the limitations of the procedure. If you really only want a one cup size increase, then it may be possible to achieve it with fat grafting. If you have enough extra fat for two grafting procedures then the outcome can be even better. Breast fat grafting is very common in my practice. It is considered a safe option for patients desiring a small change in size. The most important thing is for patients to have realistic expectations about the size increase that can be achieved with fat grafting. Thin patients may benefit from weight gain before surgery in order to have adequate donor fat for liposuction and fat grafting. Otherwise, I don't recommend that patients gain weight before fat grafting procedures.Best wishes, Dr. Connall
