It appears from your photograph that you have had a breast lift in the past. Due to the scarring and change in blood supply to the nipple/areola, I would recommend the implant be placed UNDER the muscle, not ABOVE.

For cleavage, you will need a bigger implant. The most common complaint after breast implant surgery is that the women wished they had chosen a larger implant size. Go with the bigger implant.
