First it is not your age , it’s how you age. You pursue surgery when the facial aging bothers you. There was a twins study years ago with ten year follow up which revealed that a deep plane did not provide longer lasting results at ten years.
A smas face lift will provide excellent results. The issue is how to address the smas : imbricaton, smasectomy, or high smas.

From the picture you have provided it appears your implant is not centered on your nipple. The only way to attain that is to either lower the fold or raise the nipple. More pictures front and side would help to further address your concerns.

Based on your picture, you have a great shape you just need additional volume. As you have breast tissue, a saline implant would provide a great result.
The issue with fat is the take, the amount that survives, is unpredictable.

From your pictures it appears You have some loose skin in your neck and jawline. This is early laxity. The issue is how much does it bother you? If it does, you might consider an extended s lift to address both areas. The timing is really when it bothers you.

The only time I used an expander with a LD is when I know the patient is to undergo radiation therapy. If the same expander is in place with multiple infections consider removing the expander and letting things quiet down and then come back to replace an implant.

The first question you have to answer as what size you would like to be. If you like the larger breasts I feel that it would be more attractive lifted which would give you more to look up a bra without a bra. At that point I would then put an implant on the right for symmetry purposes

Now that we are able to perform elective surgery. Patients who had been cancelled due to state limitations are now wanting to get back on the schedule. In addition many patients are choosing to have plastic surgery and recover while they are working from home. thus, no one know they had surgery and it does not count as sick leave.

If as you say you have excess skin currently under your eyes, a midface will only contribute more skin to your lower lids as it is a vertical lift. The cheek can be elevated with endoscopic dissection or under direct visualization. The skin is remove through an incision under your lashline. There may be some early changes in the shape of the eye as the lid is often tightened as a component of this procedure. This typically resolves over 6-8 weeks.

A midface is a vertical lift. The procedure is performed starting at the area just outside the corner of your eye and then heading along the rim under your eye onto your cheek. The entire cheek is released and elevated and then suspended to the area where the surgery started as well as the temproal region within your hair. Once the midface is elevated, there will be some excess skin contributed to the lower lid which should be conservatively removed with an incision under your lash line.

I do not feel you need a midface. To clarify, a midface is more of a vertical pull, not in the direction obliquely toward your ear. Prior to considering surgery, I would try a second attempt at filler both in the malar region as well as just below the corner of your mouth. This is much less involved than surgery and very well may address your concerns.

From your limited pictures, it appears that you have jowl formation and loose skin in your neck. There also appears to be a hollow in front of your jowl. These are issues which I feel are best addressed by tightening the skin of your cheeks and neck as well as the underlying muscles. In addition you may benefit by the addition of fat to the area in front of your jowl, closer to your chin. More pictures would be helpful to fully address your concerns.
