Based on your photographs I would say that an open rhinoplasty to deproject your nose would be the thing to do. I think that you have a prominent anterior nasal spine and this would need to be set back too. Perhaps your ideal is a little small for your face. It’s important to discuss this with an experienced rhinoplasty surgeon.

A deviated columella is the result of a deviated septum

It looks as if your columella is deviated because the septum above it is off to one side. Usually it is possible to straighten the lower end of the septum and align the columella. Sometimes the anterior nasal spin has to be centralised too. Grafts can be added to the columella to make it wider and drop it down out of the nose but the scarring sometimes pulls it back

It’s not uncommon for me to see patients like you who wish to rebalance their profile. An open rhinoplasty addresses the shape of the nose. Chin prominence can be reduced by setting back the bone of the chin. This is an operation done entirely within the mouth. X-rays of the jaw and teeth are needed and its important to avoid the tooth roots. The bone is sectioned and set back. It is held with plates.

Thanks for the question. I believe that with an open rhinoplasty it should be perfectly possible to reduce your nasal dorsum and refine the tip. I’m increasingly using the conservative tissue preserving approach that keeps the bridge line attachments of the nasal skeleton and sets back the nose from behind. Its important to have a consultation and review with an experienced rhinoplasty surgeon.
