If you remove your implants and do not replace them, you will lose the fullness in the upper portion of your breast. Your nipple position appears appropriate and would likely not require a lift for repositioning; however, a breast lift would help tighten any excess skin following explant. You may also want to explore options such as autoaugmentation with fat transfer following explant to help maintain some volume. Ultimately, you have a variety of options to discuss with your plastic surgeon.
Will revision surgery address my low implant?

I believe this appears as a “double bubble” where a small portion of your implant is sitting below your inframammary fold (natural crease at the bottom of the breast). Recreating your inframammary fold by reattaching your scar to the chest wall should achieve a similar appearance to the other breast. You may also consider talking with your surgeon about supporting the implant with sutures or mesh.
Will scar from mole removal improve?

After removal, the scar continues to remodel for 6 months. I would continue post-excision scar management including keeping the scar moist and protected with sunscreen. The redness generally tends to fade over time. If you were to have the area removed for a straight line, the line would be longer than the current red area, so keep that in mind when considering removal. I would work with the surgeon who performed the excision for a result you are both pleased with.

Facial surgery is always complex. The balance between surgical excision and minimizing scarring. Anytime you injure the skin a scar will form; unfortunately there is no way around this. A punch biopsy would remove the mole and likely leave just a small scar. Depending on the lesion you could also have the mole shaved off, which generally results in a less noticeable scar. Wrinkling skin is difficult to treat. Micro needling is a good option to improve skin texture. There are also specific CO2 lasers designed for hands. These are typically aimed at treating sun spots but may provide some benefit for smoothing the skin on the back of your hands. Yes, you are correct about having an indentation when you’re not smiling – while you recover from your surgery. I would recommend avoiding any filler product to the area while it is healing. The dimpleplasty procedure relies on a scar forming between the undersurface of your skin and the muscle below. Placing filler may interfere with this process and decrease the effectiveness of the procedure.

This is a challenging situation. The skin is scared to the underlying facial musculature. Fat transfer is a great option but should be performed with subcision for optimal results. Subcision could be performed by using a small cannula beneath the skin to release the skin from the underlying muscle. Unfortunately, this is not always 100% successful.
